摘要
目的探讨产前超声检查对胎儿畸形的诊断效果。方法选取102例2018年7月~2020年1月在我院行产前超声胎儿畸形筛查的孕妇作为研究对象,其均在产前行二维超声检查和四维超声检查,对本组孕妇的临床资料进行回顾性分析,并与最终临床诊断结果进行比较,统计检出的胎儿畸形发生率,对二维超声和四维超声的诊断效能进行统计学分析比较。结果经分娩后或者引产后确诊,共检出畸形胎儿18例(17.65%),其中脑部畸形1例(0.98%)、心脏畸形4例(3.92%)、四肢畸形1例(0.98%)、肾脏畸形3例(2.94%)、脊柱裂畸形3例(2.94%)、唇腭裂畸形6例(5.88%);二维超声诊断的敏感度(50.00%)、准确度(90.20%)均低于四维超声(77.78%、92.16%),且低于联合诊断结果(88.89%)、(97.06%),差异有统计学意义(P<0.05)。结论孕妇在分娩前行产前超声检查实施胎儿畸形筛查,建议以四维超声诊断结果为准,能够为临床诊断和治疗提供准确的指导依据,有较高的推广价值。
Objective To investigate the diagnostic efficiency of prenatal ultrasonography on fetal malformation.Methods A total of 102 cases of pregnant women who were screened for fetal malformation by prenatal ultrasonography admitted to our hospital from July 2018 to January 2020 were selected as the objects.All of them were given two-dimensional(2D)ultrasound and four-dimensional(4D)ultrasound before delivery.The clinical data of this group of pregnant women were analyzed retrospectively and compared with the final clinical diagnosis.The incidence of fetal malformation was counted,and the diagnostic efficiency of 2D ultrasound and 4D ultrasound was compared statistically.Results 18 cases(17.65%)were diagnosed as fetal malformation after delivery or induced labor,including 1 case(0.98%)of cerebral malformation,4 cases(3.92%)of cardiac malformation,1 case(0.98%)of limbic malformation,3 cases(2.94%)of nephritic malformation,3 cases(2.94%)of spina bifida malformation and 6 cases(5.88%)of cleft lip and palate malformation.The sensitivity and accuracy of 2D ultrasound were lower than those of 4D ultrasound(50.00%vs 77.78%,92.16%vs 90.20%),and lower than those of combined diagnosis(88.89%,97.06%),with statistically significant differences(P<0.05).Conclusion Pregnant women should be screened for fetal malformation by prenatal ultrasonography before delivery.It is suggested that the diagnosis results of four-dimensional ultrasound examination should prevail,which can provide accurate guidance for clinical diagnosis and treatment,which has relatively high value of promotion.
