产ESBLs大肠埃希菌检出率、基因型分布及其耐药性分析 被引量:5

摘要 目的:探讨产超广谱 β-内酰胺酶(ESBLs)大肠埃希菌检出率、基因型分布及其耐药性.方法:选取2018年1月-2020年1月医院细菌室分离出的82株大肠埃希菌,分析产ESBLs大肠埃希菌检出率、基因型分布情况以及耐药性.结果:82株大肠埃希菌中产ESBLs大肠埃希菌检出率为53.66%(44/82),其中43株(97.73%)为BlaCTX-M基因型,其中以BlaCTX-M-9为主,其余1株携带BlaTEM基因型,菌株为BlaTEM-1型.BlaCTX-M-9、BlaCTX-M-1对头孢曲松、头孢唑林、头孢噻肟耐药率为100%,对左氧氟沙星耐药率分别为93.10%、100%,对亚胺培南的耐药率为0;BlaCTX-M-1对头孢吡肟、氨曲南、头孢他啶耐药率为91.67%、100%、91.67%,均高于BlaCTX-M-9的37.93%、58.62%、37.93%,差异均有统计学意义(P<0.05).结论:大肠埃希菌中产ESBLs大肠埃希菌检出率较高,其中最常见分型为BlaCTX-M,并以BlaCTX-M-9型为主,其次为BlaCTX-M-1,两者对亚胺培南均不敏感,对头孢唑林、头孢曲松、头孢噻肟耐药、左氧氟沙星高度耐药,临床需根据药敏实验结果合理选择抗菌药物. Objective:To investigate the detection rate,genotype distribution and drug resistance of Escherichia coli producing extended-spectrumβ-lactamases(ESBLs).Method:A total of 82 strains of Escherichia coli isolated from the hospital’s bacteria room from January 2018 to January 2020 were selected,the detection rate,genotype distribution and drug resistance of ESBLs-producing Escherichia coli were analyzed.Result:The detection rate of 82 strains of ESBLs was 53.66%(44/82),among which 43 strains(97.73%)were BlaCTX-M genotype,most of which were BlaCTX-M-9,and the remaining 1 strain carried BlaTEM genotype and strain was BlaTEM-1.The drug resistance rates of BlaCTX-M-9 and BlaCTX-M-1 to Ceftriaxone,Cefazolin and Cefotaxime were 100%,93.10%and 100%respectively to Levofloxacin,and 0 to Imipenem.The drug resistance rates of BlaCTX-M-1 against Cefepime,Amitracin and Ceftazidine were 91.67%,100%and 91.67%,higher than 37.93%,58.62%and 37.93%of BlaCTX-M-9,with statistically significant differences(P<0.05).Conclusion:The detection rate of ESBLs producing Escherichia coli is higher,among which BlaCTX-M is the most common type,and BlaCTX-M-9 is the main type,followed by BlaCTX-M-1.Both of them are not sensitive to Imipenem,but highly resistant to Cefazolin,Ceftriaxone,Cefotaxime and Levofloxacin.It is necessary to select antibiotics reasonably according to the results of drug sensitivity test.
