摘要
目的探讨5-氟尿嘧啶/亚叶酸钙+奥沙利铂+伊立替康(FOLFOXIRI)方案应用于老年晚期胃癌姑息化疗中的临床疗效。方法采用随机数字表法将81例晚期胃癌患者随机分为研究组(n=41)与对照组(n=40)。对照组患者给予卡培他滨单药治疗,研究组患者给予FOLFOXIRI方案进行姑息化疗。比较两组患者的临床疗效、生存质量评分、不良反应发生率和免疫功能状态。结果研究组患者的总有效率为60.98%(25/41),高于对照组患者的35.00%(14/40)(P﹤0.05)。治疗后,研究组患者的生理状况、情感状况评分均低于对照组,功能状况、社会/家庭状况评分均高于对照组(P﹤0.05)。两组患者的胃肠道反应、手足综合征、骨髓抑制、中性粒细胞减少和血小板减少的发生率比较,差异无统计学意义(P﹥0.05)。治疗后,对照组患者的自然杀伤(NK)细胞、CD8^+、CD4^+、CD3^+水平均低于观察组(P﹤0.05)。结论FOLFOXIRI方案姑息化疗可有效提高老年晚期胃癌患者的临床疗效,改善患者的生存质量和免疫功能,值得临床推广。
Objective To investigate the clinical effect of 5-fluorouracil/calcium leucovorin+oxaliplatin+irinotecan(FOLFOXIRI)regimen in palliative chemotherapy for elderly advanced gastric cancer patients.Method A total of 81 patients with elderly advanced gastric cancer were randomly divided into a study group(n=41)and a control group(n=40)using the random number table method.The patients in the control group were treated with capecitabine as a single-agent regimen,and the patients in the study group were given palliative chemotherapy with FOLFOXIRI regimen.The clinical efficacy,quality of life score,the incidence of adverse events and immune function were compared between the two groups.Result The total response rate of patients in the study group was 60.98%(25/41),which was higher than 35.00%(14/40)of the control group(P<0.05).After treatment,the scores of physiological status and emotional status of the study group were lower than those of the control group,and the scores of functional status and social/family status were higher than those of the control group(P<0.05).There was no statistically significant difference in the incidence of gastrointestinal events,hand-foot syndrome,bone marrow suppression,neutropenia and thrombocytopenia between the two groups(P>0.05).After treatment,the levels of natural killer(NK)cells,CD8^+,CD4^+and CD3^+in the control group were lower than those in the observation group(P<0.05).Conclusion The application of FOLFOXIRI regimen in palliative chemotherapy of advanced gastric cancer in the elderly can effectively increase the clinical efficacy,improve the quality of life and immune function of patients,which is worthy of clinical promotion.
