摘要
目的探讨鼻内镜辅助支撑喉镜切除声带息肉手术临床应用效果。方法选择2016年8月—2019年8月本院收治的525例声带息肉患者为研究对象,根据患者是否有DLE危险因素及是否有鼻内镜辅助手术进行分组回顾性分析,比较组内及组间手术治疗效果、并发症及术后复发情况。结果DLE低危组中行单纯支撑喉镜手术和鼻内镜联合支撑喉镜手术有效率、术后并发症发生率、术后复发率分别为87.66%和95.08%、13.64%和6.56%、11.69%和4.10%;DLE高危组中行单纯支撑喉镜手术和鼻内镜联合支撑喉镜手术有效率、术后并发症发生率、术后复发率分别为65.12%和84.62%、39.53%和17.31%、30.23%和13.46%;组内、组间比较差异都有统计学意义(P<0.05)。结论鼻内镜联合支撑喉镜手术可提高手术有效率,减少手术并发症及术后复发,虽然DEL高危患者手术有效率相对偏低,手术并发症及术后复发率相对较高,但总体改善效果亦是明显的。
Objective To explore the clinical efficacy of nasal endoscopic assisted for removal of vocal cord polyps under laryngoscope.Methods A total of 525 patients with vocal cord polyps admitted to the hospital from August 2016 to August 2019 were selected as the objects of study.A group retrospective analysis was conducted according to whether the patients had DLE risk factors and whether they underwent nasal endoscopic assisted surgery,to compare the effect of surgical treatment,complications and postoperative recurrence between intra-group and inter-group.Results In the DLE low-risk group,the effective rate,rate of postoperative complication and postoperative recurrence rate of support laryngoscope group and nasal endoscope combined with laryngoscope were 87.66%and 95.08%,13.64%and 6.56%,11.69%and 4.10%,respectively.In the DLE high-risk group,the effective rate,postoperative complication rate and postoperative recurrence rate of single laryngoscope group and nasal endoscope combined laryngoscope group were 65.12%and 84.62%,39.53%and 17.31%,30.23%and 13.46%,respectively.The differences between intra-group and inter-group of the two groups were statistically significant(P<0.05).Conclusions Nasal endoscope combined with support laryngoscope could improve the effective rate of surgery,reduce surgical complications and postoperative recurrence.Despite the effective rate of surgery is relatively low in patients in high-risk DEL group,and the rate of surgical complications and postoperative recurrence is relatively high,the overall improvement effect is also significant.
