摘要
目的探讨改良双钢板内固定治疗复杂胫骨平台骨折的临床效果及对膝关节功能的影响。方法选择2016年6月至2018年6月安徽理工大学附属淮南东方医院集团总医院收治的93例复杂胫骨平台骨折患者,采用随机数字表法分为对照组(46例)与观察组(47例)。对照组采用锁定钢板内固定治疗,观察组采用改良双钢板内固定治疗,比较两组的手术时间、术中出血量、负重时间、骨折愈合时间及术后并发症发生情况,术后3个月和6个月胫骨平台内翻角(tibial plateau varus angle,TPA)和后倾角(retroversion angle,PA)的变化,以及术后6个月膝关节功能改善情况。结果两组手术时间、术中出血量、负重时间比较差异无显著性(P>0.05)。观察组骨折愈合时间短于对照组,术后并发症发生率(8.51%)低于对照组(28.26%),差异均有显著性(P<0.05)。术后3个月和6个月两组的TPA、PA比较差异均无显著性(P>0.05)。术后6个月观察组膝关节功能优良率(95.74%)高于对照组(82.61%),差异有显著性(P<0.05)。结论改良双钢板内固定对复杂胫骨平台骨折的临床疗效良好,且可改善患者膝关节功能,值得临床应用。
Objective To investigate the effect of modified double plate internal fixation on the clinical efficacy and knee function of complex tibial plateau fractures.Method The 93 patients with complex tibial plateau fractures admitted to our hospital from June 2016 to June 2018 were randomly divided into control group(46 cases)and observation group(47 cases).The patients in the control group were treated with locking plate internal fixation,while the patients in the observation group were treated with modified double plate internal fixation.The changes of surgical indicators and complications were compared between the two groups.The tibial plateau varus angle(TPA)and retroversion angle(PA)were measured at 3 months and 6 months after operation,and the knee joint function was improved at 6 months after operation.Result There was no significant difference in bleeding volume,weight-bearing time and operation time between the two groups(P>0.05);the healing time of the observation group was faster than that of the control group(P<0.05).The incidence of postoperative complications in the observation group(8.51%)was lower than that in the control group(28.26%)(P<0.05).There was no significant difference in TPA and PA between the two groups at 3 months and 6 months after operation(P>0.05).The excellent and good rate of knee joint function in the observation group(95.74%)was higher than that in the control group(82.61%)(P<0.05).Conclusion Modified double plate internal fixation has good clinical effect on complex tibial plateau fracture and can improve knee joint function,which has important clinical research value.
