摘要
目的探讨喜炎平联合高流量湿化氧吸入对急性呼吸衰竭患者炎症介质和肺功能的影响。方法参照随机分配原则将本院急诊科收治的124例急性呼吸衰竭患者分为2组,各62例。对照组采用高流量湿化氧吸入疗法;研究组在对照组基础上采用喜炎平注射液。采用酶联免疫吸附试验(ELISA)检测血清降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、白细胞介素(IL)-4、IL-10、IL-27和肿瘤坏死因子(TNF)-α,采用FLEXO型呼吸机检测第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、潮气量(TV)。结果研究组治疗总有效率92%,明显高于对照组的79%(P<0.05)。治疗前,2组炎症介质比较差异无统计学意义(P>0.05),治疗后,2组上述指标较治疗前明显降低,研究组明显低于对照组(P<0.05)。治疗前,2组FEV1、FVC、FEV1/FVC、TV比较差异无显著性(P>0.05),治疗后,2组FEV1、FEV1/FVC和TV明显高于治疗前,研究组明显高于对照组,P<0.05。2组药物不良反应发生率比较差异无统计学意义(P>0.05)。结论喜炎平联合高流量湿化氧吸入对急性呼吸衰竭患者炎症介质和肺功能的改善作用显著,且药物安全性较高,值得临床推广使用。
Objective To investigate the effects of Xiyanping combined with high-flow humidified oxygen inhalation on inflammatory mediators and lung function in patients with acute respiratory failure.Methods A total of 124 patients with acute respiratory failure admitted to the emergency department of our hospital were randomly included in the study and divided into two groups(n=62 each).The control group was treated with high-flow humidified oxygen inhalation.In addition,the control group was treated with Xiyanping injection.The levels of serum procalcitonin(PCT),hypersensitive C-reaction protein(hs-CRP),interleukin-4(IL-4),interleukin-10(IL-10)and interleukin-27(IL-27)and tumor necrosis factor-α(TNF-α)were examined by ELISA.The forced expiratory volume in the first second(FEV1),forced vital capacity(FVC)and tidal volume(TV)were determined by FLEXO.Results The total effective rate in the study group was significantly higher than that in the control group(92%vs.79%,P<0.05).There were no statistically significant differences in inflammatory mediators between the two groups at baseline(all P>0.05).After the treatment,the levels of the indicators significantly decreased,and the levels in the study group were significantly lower than those in the control group(P<0.05).There were no significant differences in the levels of FEV1,FVC,FEV1/FVC,and TV between the two groups at baseline(all P>0.05).After the treatment,the levels of FEV1/FVC and TV in the two groups were significantly higher than those at the baseline,and the levels in the study group were significantly higher than those in the control group(all P<0.05).There was no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion Xiyanping combined with high-flow humidified oxygen inhalation is significantly effective in the improvement of inflammatory mediators and lung function in patients with acute respiratory failure,and the drug safety is higher,which justifies wide spread us in the clinical settings.
