摘要
目的:研究肺癌立体定向放射治疗患者在CBCT扫描后进行六维方向的配准相对于传统四维方向配准在位移误差纠正方面的必要性及其临床意义。方法:选取2018年3月~2019年7月在本院接受立体定向放射治疗的肺癌患者25例,使用VARIAN的edge直线加速器先进行CBCT扫描,再分别在四维方向和六维方向与计划中的定位CT图像进行配准,并记录数据进行统计学分析。结果:所有接受治疗的患者共获取204组对比数据,其中以六维方向配准而获得的腹背方向的位移误差为(-1.15±0.23)mm、头脚方向的位移误差为(-0.53±0.24)mm、左右方向的位移误差为(0.37±0.18)mm,水平旋转方向误差为(-0.05±0.07)˚;而以四维方向配准所获得的腹背方向的位移误差为(-1.22±0.22)mm、头脚方向的位移误差为(-0.43±0.26)mm、左右方向的位移误差为(0.31±0.17)mm,水平旋转方向误差为(-0.02±0.06)˚四者P值分别为0.840、0.764、0.822以及0.784,没有统计学意义。而在六维方向配准还有俯仰方向以及左右旋转方向的数据,此二者的角度偏移误差分别为(0.32±0.05)˚、(0.17±0.07)˚,由于四维配准在此二轴方向皆为0˚,所以两者P值为0,有着显著的统计学意义。结论:肺癌立体定向放射治疗患者,六维配准对于患者体位的位移误差纠正的精确性有着显著的优势,以及配合六维床可以直接对六个方向进行纠正后治疗,能更好地保持治疗时的体位。
Objective:To research the necessity and clinical significance of the registration of six-dimensional directions after CBCT scanning in patients with lung cancer stereotactic radiotherapy in relation to the correction of displacement error in traditional four-dimensional alignment.Methods:Twenty-five patients with lung cancer who underwent stereotactic radiotherapy from March 2018 to July 2019 were enrolled in the CBCT scan using the VARIAN edge linear accelerator,Then registration with the planned CT image in the 4D and 6D directions respectively.Results:All the patients who received treatment received a total of 204 sets of comparative data.The displacement error in the ventral and dorsal direction was(-1.15±0.23)mm and the displacement error in the head and foot direction was(-0.53±0.24)mm.The displacement error of mm and left and right directions is(0.37±0.18)mm,the error of horizontal rotation direction is(-0.05±0.07)˚,and the displacement error of the ventral direction obtained by registration in four dimensions is(-1.22±0.22)mm The displacement error in the direction of mm and head and foot is(-0.43±0.26)mm,the displacement error in the left and right direction is(0.31±0.17)mm,and the error in the horizontal rotation direction is(-0.02±0.06)˚.The P values of the four are 0.84,respectively.0.76,0.82,and 0.78 were not statistically significant.In the six-dimensional direction,there are also data of the pitch direction and the left and right rotation directions.The displacement errors of the two are(0.32±0.05)˚and(0.17±0.07)˚,respectively,because the four-dimensional registration is in both directions.It is 0˚,so the P values of both are 0,which are statistically significant.Conclusion:In patients with stereotactic radiotherapy for lung cancer,six-dimensional registration has a significant advantage in correcting the displacement error of the patient’s position,and the six-dimensional bed can directly correct the treatment in six directions which can better maintain the body position during the treatment.
