摘要
目的本研究在对X社区卫生服务中心签约服务现状研究的基础上,总结我国现阶段家庭医生签约服务费的典型筹资模式,从契约理论的角度出发分析不同主体之间的博弈关系,提出有关签约服务费的政策建议,以期为规范该项工作提供科学依据。方法政策分析、文献分析、访谈法、实地调研法。结果X社区卫生服务中心存在两个方面疑惑:一是目前医疗收费项目里不含签约服务费,二是该地区医保资金对签约服务费不支持。现阶段我国主要存在2种医保支持签约服务费的典型模式,一种是支持基础签约服务,另一种是支持个性化签约服务,我国部分地区的经验值得借鉴和思考。结论我国各部门应该协同合作,从筹资渠道、筹资标准、分配机制、补偿机制、资金监管等方面进一步完善医保支付签约服务费的模式。
Objective This paper summarizes the typical financing mode of family doctor signing service fee in our country,based on the analysis of the contracted service status of X community health service center.Then,from the perspective of contract theory,this paper analyzes the game relations between different subjects and puts forward some policy suggestions on contract service fee,so as to provide scientific basis for standardizing this work.Methods policy analysis,literature analysis,interview,field research Results There are two difficulties in X community health service center:first,there is no signing service fee in the current medical fee project,and second,the region's medical insurance funds do not support signing service fee.At the present stage,there are two typical modes of supporting the signing service fee of medical insurance in China,one is to support the basic signing service,the other is to support the personalized signing service.The experience of some regions in China is worth learning from and thinking about.Conclusions Different departments in China should work together to further improve the mode of signing up service fees for medical insurance from the aspects of financing channels,financing standards,distribution mechanism,compensation mechanism and fund supervision.
