摘要
目的:分析心脑血管疾病患者的住院费用及其影响因素,为降低患者经济负担、控制医疗费用的过快增长提供参考。方法:对2017年吉林省某三甲医院30953例心脑血管疾病患者的住院费用进行描述性分析、相关性分析、单因素分析和多元线性回归分析。结果:在所调查的心脑血管疾病患者中,冠心病患者最多,脑血栓患者的次均住院费用最高。与住院费用相关性最高的是治疗费。性别、年龄、是否参保、住院天数和是否手术均为住院费用影响因素。住院天数(标准回归系数为0.577)和是否手术(标准回归系数是0.590)对住院费用的影响很大。结论:应加强对老年群体以及男性居民心脑血管疾病防治知识的宣传;科学缩短住院天数,规范临床路径;推动分级诊疗制度与医保政策融合,合理引导患者就医选择。
Objective:To analyze the hospitalization cost of cardiovascular and cerebrovascular patients,explore the factors and to provide references for reducing the economic burden of patients and controlling the excessive growth of medical expenses.Methods:Descriptive analysis,correlation analysis,single factor analysis and multiple linear analysis were conducted on the hospitalization cost of patients with cardiovascular and cerebrovascular diseases in a grade-A tertiary hospital in Jilin province in 2017.Results:Among the investigated patients with cardiovascular and cerebrovascular diseases,the patients with coronary heart disease were the most common,and the average hospitalization cost for cerebral thrombosis was the highest.The highest correlation with hospitalization costs was treatment costs.Gender,age,insured or not,length of stay,and whether getting surgery were all influencing factors that affected hospitalization costs.The length of stay(standard regression coefficient was 0.577)and whether getting surgery(standard regression coefficient was 0.590)had a great influence on hospitalization expenses.Conclusion:It is suggested that attention should be paid to raise the awareness and popularization of the prevention and treatment of cardiovascular and cerebrovascular diseases among the middle-aged and elderly groups as well as male residents,and reasonably shorten the length of hospitalization without reducing the therapeutic effect of medical services.And it is important to promote the integration of hierarchical medical system and medical insurance policy,and to rationally guide the patients'choice of medical treatment.
