摘要
目的:研究血液透析(hemodialysis,HD)联合血液灌流(hemoperfusion,HP)对老年终末期糖尿病肾病(end stage diabetic kidney disease,ESDKD)患者胰岛素抵抗(insulin resistance,IR)及生活质量的影响。方法:选取2018年2月-2019年2月于南京医科大学附属江宁医院血液净化中心透析的老年糖尿病肾病(diabetic kidney disease,DKD)患者共76例,随机分为两组,分别为HD组、HD+HP组,各38例,每半年监测一次,共1年时间。比较两组治疗前后血清空腹胰岛素(FINS)、空腹血糖(FPG)、胰岛素抵抗指数(HOMA-IR)、C反应蛋白(CRP)及生活质量。结果:与治疗前相比,HD+HP患者的CRP、HOMA-IR水平均显著低于HD组(P<0.05),而生活质量评分(SF-36)总分显著高于HD组(P<0.05)。结论:HD联合HP治疗在改善IR方面的效果明显优于单纯HD治疗,能减轻DKD患者体内微炎症状态,可能有提高患者生活质量优势。
Objective:To study effects of hemodialysis (HD) combined with hemoperfusion (HP) on insulin resistance and quality of life for elderly patients of end-stage diabetic kidney disease (DKD).Method:A total of 76 patients with diabetic kidney disease maintaining hemodialysis from February 2018 to February 2019 in Jiangning Hospital Affiliated to Nanjing Medical University were randomly divided into two groups.There were 38 cases in each.HD group and HD+HP group respectively,which were monitored every half year for a total of 1 year.Fasting insulin (FINS),fasting blood glucose (FPG),insulin resistance index (HOMA-IR),serum C-reactive protein (CRP) and quality of life were compared between the two groups before and after treatment.Result:Compared with before treatment,the levels of CRP and HOMA-IR in the HD+HP group were significantly lower than those in the HD group (P<0.05),and the total scores of quality of life (SF-36) was significantly higher than the HD group (P<0.05).Conclusion:HD combined with HP therapy is significantly better than HD therapy alone in improving insulin resistance in hemodialysis patients.It can reduce diabetic nephropathy and maintain microinflammatory status.There may be advantages to improving patients’ quality of life.
