摘要
目的探讨应用纳米银敷料联合表皮生长因子凝胶在小面积骨外露创面换药中的临床疗效。方法选择27例小面积(<5 cm^(2))骨外露创面患者,其中男性16例,女性11例;年龄18~88岁,中位年龄56岁。均采用纳米银浸透的多层敷料结合表皮生长因子凝胶为其换药,换药过程中均在前、中、后3期送细菌培养,记录细菌培养阳性率。对其创面愈合情况进行随访观察,记录创面最终的愈合时间及愈合效果。结果所有病例均获得2~6个月随访,平均随访时间(3.21±1.62)个月。27例患者创面均愈合,愈合时间6~12周,平均愈合时间8.62周。愈合后创面外形满意,无骨外露,无红肿渗液,无窦道及骨髓炎发生,达到一期愈合。结论应用纳米银敷料及表皮生长因子凝胶换药,在抗感染的同时可有效促进创面肉芽的生长,最终达到创面愈合的目的,是一种有效的治疗小面积骨外露创面的方法。
Objective To investigate the clinical effect of nano-silver dressing combined with epidermal growth factor gelatum in dressing of small bone exposure wound.Methods A total of 27 patients with small bone exposure wound(<5 cm^(2))were enrolled,which included 16 males and 11 females,aged 18-88 years old with median age of 56 years old.The multi-layer dressing impregnated nano-silver combined with epidermal growth factor gel dressing was performed.The bacteria of before,during and after dressing were cultured,and positive rates of bacterial culture were recorded.Wound healing was followed up,healing time and effect were recorded.Results All cases were followed up for 2-6 months with mean follow-up of(3.21±1.62)months.All wounds were healed,the healing time was 6-12 weeks with mean healing time of 8.62 weeks.After healing,the appearance of wound was satisfactory without bone exposure,swelling and exudation,sinus,osteomyelitis,and primary healing was achieved.Conclusion It is demonstrated that nano-silver dressing and epidermal growth factor gel dressing could effectively promote growth of wound granulation and achieve wound healing while resisting infection.It is an effective method for treatment of small-area bone exposed wounds.
