摘要
目的:结合临床实际,优化处理处方前置审核系统中临床意义不明确的相互作用拦截,保障临床合理用药。方法:通过对处方前置审核系统静默2个月运行期中拦截到的所有案例进行人工筛查,利用UpToDate中Lexicomp相互作用数据库,以及PubMed、马丁代尔药典、最新临床指南等数据库查找循证医学依据,并与临床医生进行沟通和反馈,确定为具有临床意义的药物相互作用则保留红灯或黑灯拦截。结果:系统共拦截到药物相互作用问题620个,其中186个(30%)缺少足够的循证依据或未发生相互作用,可以取消拦截或者降低提醒级别。结论:处方前置审核系统中内置相互作用审查规则,需药师进行全面人工筛查,根据最新循证依据以及临床用药实际情况,做出符合临床用药习惯的特有知识系统数据库,保障临床合理用药。
Objective: To optimize the computerized drug-drug interaction( DDI) alerts without definite clinical results in pre-prescription review system according to the clinical feature in order to ensure safe medication. Methods: All the DDIs intercepted by preprescription review system during two months of the quiet period were screened manually,and the evidence of evidence-based medicine was searched according to the Lexicomp database attached to Up To Date,PubMed database,Martindale’s Pharmacopoeia and the latest treatment guidelines. The DDIs with clinic significance were maintained red or black light interception after communicating with clinicians and obtaining feedback. Results: Totally 620 DDI cases occurred in the pre-prescription review system,and 186( 30%) of which had no enough evidence of evidence-based medicine or without interaction at all,suggesting the interception be cancelled or reduced the alert level. Conclusion: Drug-drug interaction review rules are built in pre-prescription review system,which requires pharmacists conducting comprehensive manual screening. According to the latest evidence-based medicine and the actual situation of clinical medication,a unique knowledge system database should be made to meet clinical medication habits,so as to ensure safe and rational medication.
