摘要
目的:总结儿童操作性疼痛常见非药物疗法的最佳证据,为临床实践提供参考。方法:计算机检索2010年4月6日—2020年4月16日UpToDate、循证卫生保健中心数据库(JBI)、英国国家临床医学研究所指南库(NICE)、苏格兰院际指南网(SIGN)、医脉通指南网、加拿大安大略注册护士协会(RNAO)、美国疼痛协会(APS)、国际疼痛研究协会(IASP)、Cochrane Library、荷兰医学文摘数据库(EMbase)、护理文献累积索引数据库(CINAHL)、PubMed、中国生物医学文献数据库(CBM)、中国知网(CNKI)、万方数据库(Wanfang)及相关指南网站中关于儿童操作性疼痛常见非药物疗法的临床实践指南、证据总结、临床决策与系统评价,2名研究者根据统一标准独立评价文献质量并提取最佳证据。结果:共纳入文献15篇,其中临床实践指南4篇,证据总结2篇,临床决策1篇,系统评价8篇,最终总结出21条最佳证据,涉及母乳喂养、袋鼠式护理、甜味剂、非营养性吮吸、便利蜷曲与襁褓包裹、分散注意力及结合使用非药物疗法7个方面。结论:非药物疗法安全性高且护士可独立操作,临床护士可综合患儿年龄、科室环境等因素,推动证据的转化与应用,及时关注证据更新情况,以减轻儿童操作性疼痛。
Objective:To summarize the best evidence for common non-pharmaceutical therapy of procedural pain in children,and to provide references for clinical practice.Methods:The clinical practice guidelines,evidence summaries,clinical decisions and systematic reviews for common non⁃pharmaceutical therapy of procedural pain in children were retrieved by computer in UpToDate,JBI,NICE,SIGN,RNAO,APS,IASP,Cochrane Library,EMbase,CINAHL,PubMed,CBM,CNKI,Wanfang databases and related guideline websites from April 16,2010 to April 16,2020.Two researchers evaluated the literature quality independently according to the unified standard and extracted the best evidences.Results:A total of 15 literatures were extracted,including four clinical practice guidelines,two evidence summaries,one recommended practice and eight systematic reviews.Finally,21 pieces of evidence were summarized,involving seven aspects,which were breast feeding,kangaroo nursing,sweetener,non⁃nutritive sucking,facilitated tucking and swaddling,distraction and combined non⁃pharmaceutical therapy.Conclusion:Non⁃pharmaceutical therapy was safe and could be operated independently by nurses.Clinical nurses could promote the transformation and application of evidence according to the age of children and department environment,and timely pay attention to the update of evidence to alleviate children's procedural pain.
