摘要
目的探讨保乳手术在早期乳腺癌治疗中的疗效与安全性。方法根据手术方法的不同将83例早期乳腺癌患者分为对照组(n=41)和观察组(n=42),对照组患者采用乳腺癌改良根治术,观察组患者采用保乳手术。比较两组患者的手术指标、并发症发生情况、乳房外观、生活质量及预后。结果观察组患者的切口长度、手术时间、住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P﹤0.01)。观察组患者的并发症总发生率为11.9%,明显低于对照组的48.8%,差异有统计学意义(P﹤0.01)。术后观察组患者的乳房外观优良率为85.7%,明显高于对照组的41.5%,差异有统计学意义(P﹤0.01);术后观察组患者的生理状态、心理状态、社会功能、环境情况评分均明显高于对照组,差异均有统计学意义(P﹤0.01)。两组患者术后6个月、1年的复发率、远处转移率、生存率比较,差异均无统计学意义(P﹥0.05)。结论保乳手术治疗早期乳腺癌的创伤较小,手术时间较短,缩短了患者的住院时间,降低了并发症发生率,提高了乳房外观优良率,且其预后与乳腺癌改良根治术并无明显差异,值得在临床中推广应用。
Objective To investigate the efficacy and safety of breast-conserving surgery in the treatment of early breast cancer.Method 83 patients with early breast cancer were divided into the control group(n=41,treated with modified radical mastectomy)and the observation group(n=42,treated with breast-conserving surgery),and the surgical indicators,complications,breast appearance,quality of life and prognosis were compared between the two groups.Result The incision length,operation time,hospitalization time and intraoperative blood loss of the observation group were significantly shorter or lower than those of the control group(P<0.01).The incidence of complications in the observation group(11.9%)was significantly lower than that in the control group(48.8%)(P<0.01).After surgery,the excellent rate of breast appearance(85.7%)was significantly higher than that of the control group(41.5%)(P<0.01);the scores of physiological state,psychological state,social function and environmental conditions in the observation group were significantly higher than those in the control group(P<0.01).There were no significant differences in 6-month and 1-year recurrent rate,distant metastasis rate and survival rate between the two groups(P>0.05).Conclusion In the treatment of patients with early breast cancer,breast-conserving surgery has less damage,shorter operation time,shortened hospitalization time,lower complications,higher excellent rate of breast appearance,without significant difference in prognosis compared with modified radical mastectomy,which is worthy of clinical promotion.
