摘要
目的研究糖尿病细菌性肝脓肿患者合并胆道系统疾病特点,探寻糖尿病细菌性肝脓肿的危险因素.方法回顾性分析2010年1月至2018年1月住院治疗的肝脓肿患者173例,所有的病例均经过临床确诊为细菌性肝脓肿,对资料进行总结分析.结果所有的肝脓肿患者合并的危险因素疾病排列依次为糖尿病、胆道疾病、肺部及其他感染性疾病、败血症,其百分比例分别为36.42%、31.79%、21.39%和12.72%(P<0.001),前两者均比后两者高(P<0.05);胆道感染类疾病是糖尿病和非糖尿病组肝脓肿患者共同合并的主要类别的胆源性疾病(11.11%比12.73%,P≥0.05).各种原因导致的无症状性胆道结构和功能改变类疾病是糖尿病组肝脓肿患者合并的主要类别的胆源性疾病,但同样的情况并未在非糖尿病肝脓肿组患者中发生(14.29%比5.45%,P<0.05).结论无症状性胆道结构和功能改变类疾病与胆道感染类疾病一样,同样可能增加糖尿病患者肝脓肿的风险.
Objective To study the characteristics of biliary tract diseases in patients of pyogenic liver abscesses with diabetes mellitus and explore the risk factors of them. Method A retrospective analysis of 173 cases of liver abscess hospitalized from January 2010 to January 2018 was conducted. All cases were clinically diagnosed as pyogenic liver abscesses. The data were summarized and analyzed. Results The sequence of all risk factors of patients with liver abscess were diabetes mellitus, biliary tract disease, pulmonary and other infection, septicemia, and the percentages were 36.42%,31.79%,21.39% and 12.72% respectively(P<0.001).Meanwhile the former two were higher than the latter(P<0.05). Biliary tract infections were the main types of biliary trac diseases associated with liver abscess in both diabetic and non-diabetic groups(11.11% vs 12.73,P≥0.05). The asymptomatic biliary tract structural and functional changes caused by various causes were the major categories of biliary trac diseases in the diabetic group with liver abscess, but the same condition did not occur in patients with non-diabetic liver abscess(14.29 % vs. 5.45 %,P<0.05).Conclusions Asymptomatic biliary tract structural and functional changes may increase the risk of liver abscess in diabetic patients as well as biliary tract infection diseases.
