摘要
目的探讨彩超引导下精准深静脉穿刺置管术在血液透析患者中的应用价值和安全性。方法选取2018—2019年于三明市第二医院肾内科行血液透析的患者80例,按照随机数字表法分为对照组和观察组,各40例。对照组行体表解剖标志深静脉,观察组行超声引导深静脉穿刺。比较2组置管时间、穿刺次数、首针穿刺成功率、置管成功率、更换穿刺部位比例、出血量、并发症发生情况、住院费用、深静脉置管后血流效果。结果观察组首次穿刺成功率、置管成功率高于对照组,置管时间、穿刺次数、更换穿刺部位比例、出血量低于对照组(P<0.05)。2组住院费用比较,差异无统计学意义(P>0.05)。观察组并发症发生率低于对照组,导管血流优良率高于对照组(P<0.05)。结论彩超引导下精准深静脉穿刺置管穿刺成功率高,耗时短,并发症少,血流量好,且未明显增加费用,是一种安全、方便、经济的方法。
Objective To investigate the application effect of color doppler ultrasound-guided precise deep vein catheterization in hemodialysis patients.Methods A total of 80 cases of hemodialysis patients were selected from 2018 to 2019 in the Department of Nephrology,the Second Hospital of Sanming,which were randomly divided into control group and research group according to random number table,40 cases in each group.The control group was treated with anatomic marking of deep vein,and the observation group was treated with ultrasound-guided deep vein puncture.The time of catheterization,times of puncture,success rate of first puncture,success rate of catheterization,proportion of replacement of puncture site,blood loss,incidence of complications,hospitalization expenses and blood flow effect after deep vein catheterization were compared between the two groups.Results The first puncture success rate and catheterization success rate of the observation group were higher than the control group,while the catheterization time,puncture times,the proportion of puncture site replacement and blood loss in the observation group were lower than the control group(P<0.05).There was no significant difference in hospitalization expenses between the two groups(P>0.05).The incidence of complications in the observation group was lower than the control group,and the excellent and good rate of catheter blood flow in the observation group was higher than the control group(P<0.05).Conclusion It is a safe,convenient and economical method with high success rate,short time consumption,few complications,good blood flow and no obvious increase in cost.
