摘要
目的:探讨玻璃体切除空气填充术治疗视网膜脱离的临床预后。方法:选择本院2018年10月~2019年6月收治的20例视网膜脱离行玻璃体切除空气填充术患者作为研究对象,随访患者治疗效果。结果:术后第1天玻璃体腔空气存留量为70%~80%,术后3~4d空气在玻璃体腔吸收至1/2,术后9~10d玻璃体腔气体吸收完全。末次随访所有患者眼部解剖复位情况完好,复位率100%。患者术后末次随访时眼压及视力均较术前改善,有统计学差异(P<0.05)。术后4例患者出现脉络膜脱离,经口服糖皮质激素治疗后逐渐消退。结论:玻璃体切除空气填充术治疗位于上方、未超过3个象限的视网膜脱离临床预后良好,术后解剖复位良好,同时患者术后眼压及视力明显改善,少数脉络膜脱离并发症患者经对症处理后恢复正常,手术治疗效果显著。
Objective:Explore the clinical prognosis of vitrectomy and air filling in the treatment of retinal detachment.Methods:20 patients with retinal detachment who were in our hospital from October 2018 to June 2019 were selected as the study subjects,follow up the treatment effect of patients.Results:Air absorption in vitreous cavity was 70%-80%on the first day after operation,air absorption in vitreous cavity was 1/2 in 3-4 days after operation,air absorption in vitreous cavity was completely absorbed 9-10 days after operation.At the last follow-up,all eyes were anatomically reduced with a reduction rate of 100%.The intraocular pressure and visual acuity of the patients were improved at the last follow-up(P<0.05).After surgery,choroidal detachment occurred in 4 patients,oral glucocorticoids improved.Conclusion:The prognosis of vitrectomy and air filling is good in the treatment of retinal detachment with above and less than 3 quadrants,after the operation,the anatomical reduction was good,intraocular pressure and vision improved significantly,a few patients with complications of choroidal detachment recovered to normal after symptomatic treatment,and the effect of surgical treatment is significant.
