摘要
目的:增强CT联合肠镜检查对直肠及直乙交界癌诊断价值。方法:选取2017年4月至2019年11月本院收治且经病理或临床活检确诊的87例结直肠癌患者作为研究对象。所有患者均进行CT与肠镜检查,观察患者病变血供、肠腔内及肠壁的变化等特征。并比较增强CT、结肠镜检查诊断直肠及直乙交界癌的符合率。结果:增强CT诊断直肠及直乙交界癌符合率为88.51%;结肠镜检查诊断符合率为89.66%;两者联合检查诊断符合率为97.70%,明显高于单一增强CT、结肠镜检查(P<0.05)。CT可见43腔内肿块,管壁明显增厚见隆起软组织密度影,向腔内凸起;19例病变脂肪间隙可见条索状或结节样影,浆膜面较模糊,增强扫描三期强化明显。发生在直乙交界处的4例病灶经结肠镜检查未诊断出,对于2例发生在直肠、2例发生在乙状结肠出的病变定位不准确。结论:增强CT、结肠镜检查均可有效诊断直肠及直乙交界癌,但两者联合检查可取长补短,且诊断符合率更高,对拟定的治疗方案提供重要的参考价值。
Objective:To analyze the diagnostic value of enhanced CT combined with colonoscopy in the diagnosis of rectal and borderline carcinoma.Methods:Eighty-seven patients with colorectal cancer who were admitted to this hospital from April 2017 to November 2019 and were diagnosed by pathology or clinical biopsy were selected as the research objects.All patients underwent CT and colonoscopy to observe the changes in diseased blood supply,intestinal lumen and intestinal wall of the disease.And compare the coincidence rate of enhanced CT and colonoscopy in the diagnosis of rectal and borderline cancer.Results:The coincidence rate of enhanced CT in diagnosing rectal and borderline carcinoma of the rectum and borderline B was 88.51%;the diagnosis coincidence rate of colonoscopy was 89.66%;the diagnosis coincidence rate of the two combined examinations was 97.70%,which was significantly higher than that of single enhanced CT and colonoscopy(P<0.05).CT showed 43 intracavitary masses,the tube wall was obviously thickened,and the raised soft tissue density shadow was bulging into the cavity;19 cases of diseased fat spaces showed cord-like or nodular shadows,and the serosal surface was blurred.Enhanced scan three-phase enhancement obvious.The 4 lesions that occurred at the junction between the straight and the second were not diagnosed by colonoscopy,and the location of the lesions that occurred in the rectum in 2 cases and the sigmoid colon in 2 cases was inaccurate.Conclusion:Both enhanced CT and colonoscopy can effectively diagnose rectal and borderline cancers,but the combined examination of the two can complement each other,and the diagnostic coincidence rate is higher,providing important reference value for the proposed treatment plan.
