盆底肌电刺激联合生物反馈对子宫全切术患者盆底肌肉肌力、 压力性尿失禁发生率及性功能的影响 被引量:10

摘要 目的研究盆底肌电刺激联合生物反馈对子宫全切术患者性功能的影响。方法选取2017年6月至2018年6月江苏省溧阳市人民医院收治的122例子宫全切术患者作为研究对象。根据患者术后治疗意愿,将其分为A组(盆底肌电刺激+生物反馈,n=62)与B组(健康宣教,n=60),术后随访6个月,观察两组的治疗效果。结果两组术前及术后3个月盆底肌肉肌力分级比较,差异无统计学意义(P>0.05);术后6个月,A组盆底肌肉肌力>3级占比显著高于B组,差异具有统计学意义(P<0.05)。两组术前及术后3个月压力性尿失禁(SUI)发生率比较,差异无统计学意义(P>0.05);术后6个月,A组SUI发生率显著低于B组,差异具有统计学意义(P<0.05)。两组术前、术后3个月及术后6个月血清松弛素水平组内比较,差异具有统计学意义(P<0.05);两组术前及术后3个月血清松弛素水平组间比较,差异无统计学意义(P>0.05);术后6个月,A组血清松弛素水平显著低于B组,差异具有统计学意义(P<0.05)。两组术前、术后3个月及术后6个月情感因素、伴侣因素得分比较,差异无统计学意义(P>0.05);两组术前、术后3个月及术后6个月生理因素及总分比较,差异具有统计学意义(P<0.05);术后6个月,B组生理因素及总分均显著低于A组,差异具有统计学意义(P<0.05)。结论全子宫全切术患者术后行盆底肌电刺激联合生物反馈能有效提高患者盆底肌肉肌力,降低SUI发生率,改善患者性生活质量,值得临床推广。 Objective To investigate the effect of pelvic electrical stimulation combined with biofeedback on sexual function in patients undergoing total hysterectomy.Methods 122 patients with total hysterectomy admitted to Liyang People′s Hospital from June 2017 to June 2018 were included,and divided into group A(pelvic floor electromyographic stimulation+biofeedback,n=62)and group B(health education,n=60)according to the postoperative treatment willingness.The patients were followed up for 6 months to observe the treatment effect of the two groups.Results There was no significant difference in pelvic floor muscle strength between the two groups before and 3 months after surgery(P>0.05).6 months after the operation,the proportion of pelvic floor muscle strength>grade 3 in group A was significantly higher than that in group B(P<0.05).There was no significant difference in the incidence of stress urinary incontinence(SUI)between the two groups before and 3 months after surgery(P>0.05).6 months after surgery,the incidence of SUI in group A was significantly lower than that in group B(P<0.05).There were significant differences in serum relaxin levels between group A and group B at the time points of pre-operation,3 months after operation and 6 months after operation(P<0.05).There was no significant difference in serum relaxin levels between the two groups before and 3 months after surgery(P>0.05).At 6 months after the operation,the level of serum relaxin in group A was significantly lower than that in group B(P<0.05).There were no significant changes in the scores of emotional factors and partner factors between the two groups before surgery,3 months and 6 months after surgery(P>0.05),and the physiological factors and total scores at each time point were significantly different(P<0.05),which were lower in the group B than in group A at the postoperative 6 months(P<0.05).Conclusions Application of pelvic electrical stimulation combined with biofeedback after total hysterectomy can effectively improve the muscle strength of the pelvic floor muscles,reduce the incidence of SUI,and improve the quality of sexual life of patients,which is worth clinical promotion.
