摘要
目的探讨"撬顶旋转复位"法联合钛针内固定治疗儿童桡骨颈骨折的临床疗效。方法回顾性分析自2016-01—2018-01采用"撬顶旋转复位"联合钛针髓内固定技术治疗的26例儿童桡骨颈骨折,术后选择长臂屈肘90°石膏托外固定。结果 26例均顺利完成手术并获得完整随访,随访时间6~12个月,平均7.1个月。手术时间20~40 min,平均32.6 min。术中出血量1~3 mL,平均量1.6 mL。术中均未出现神经、血管损伤,术后切口均获得一期愈合,未出现断针、皮下针尾处激惹、肌腱断裂等情况。骨折愈合时间6~10周,平均8.2周。末次随访时按Metaizeau标准评价疗效:优21例,良5例;按肘关节Mayo功能标准评价疗效:优22例,良3例,可1例。结论 "撬顶旋转复位"法联合钛针内固定治疗儿童桡骨颈骨折操作简便,术后并发症少、肘关节功能恢复良好,值得在临床中推广。
Objective To explore the clinical effect of"pry top rotation reduction"method combined with titanium needle internal fixation in the treatment of children radial neck fractures.MethodsA retrospective analysis was made on 26 cases of radial neck fracture in children treated with"pry top rotation reduction"combined with titanium needle intramedullary fixation technique from January 2016 to January 2018. The long arm flexion elbow flexion 90° plaster support external fixation was selected after operation.ResultsAll the 26 cases completed the operation successfully and were followed up for 6 to 12 months(mean 7.1 months). The operation time was 20 to 40 min, and the average operation time was 32.6 min. The intraoperative blood loss was 1 to 3 m L, and the average blood loss was 1.6 mL. There was no nerve or vascular injury during the operation, and all the incisions healed in one stage after operation, and there were no needle breakage, subcutaneous needle tail irritation and tendon rupture. The fracture healing time was 6 to 10 weeks, with an average of 8.2 weeks. At the last follow-up, the curative effect was evaluated according to Metaizeau criteria:excellent in 21 cases and good in 5 cases. According to the functional standard of Mayo of elbow joint, the curative effect was excellent in 22 cases, good in 3 cases and fair in 1 case.ConclusionThe"pry top rotation reduction"method combined with titanium needle internal fixation for the treatment of children radial neck fractures is easy to operate, with few postoperative complications and good recovery of elbow joint function. It is worthy of clinical promotion.
