摘要
目的分析根治幽门螺杆菌(Hpylori)感染对学龄期儿童生长发育及视黄醇结合蛋白(RPB)、铁蛋白(SF)、胰岛素样生长因子1 (IGF-1)水平的影响。方法选取2016年1月-2017年4月在绍兴市人民医院初次接受治疗的Hpylori感染患儿124例作为观察组,选取同期在本院体检健康儿童100例作为对照组。观察组患儿行一线抗Hpylori感染治疗,记录患儿临床症状分布情况,记录两组儿童胸围、体质量、身高,检测其血清锌(Zn)、血红蛋白、铁(Fe)、血尿素氮、白蛋白、RPB、SF、IGF-1含量。依据观察组患儿治疗情况将其分成Hpylori根治组99例与Hpylori复发组25例,观察两组患儿治疗前及随访2年时营养指标和生长发育状况,血清IGF-1含量和患儿BMI行相关分析。结果 124例患儿中37例有腹胀、111例有腹痛、41例睡眠欠佳、40例泛酸、85例有纳差,患儿在完成治疗后其临床症状均消失。观察组患儿体质量、身高、血清Zn、血红蛋白、Fe、血尿素氮、白蛋白、RPB及SF含量低于对照组,IGF-1含量高于对照组,差异有统计学意义(P<0.05)。随访2年后Hpylori根治组患儿体质量、身高、Zn、血尿素氮、白蛋白、RPB及SF含量高于Hpylori复发组,IGF-1含量低于Hpylori复发组,差异有统计学意义(P<0.05)。观察组患儿血清IGF-1含量和BMI为正相关(r=0.441,P<0.05)。结论 Hpylori感染会对患儿生长发育及血清营养指标产生影响,及时行Hpylori感染根治治疗可显著改善患儿营养情况与生长发育。
Objective To analyze the effect of radical treatment of Helicobacter pylori (Hp) infection on growth and development,retinol conjugated protein (RPB),serum ferritin (SF),and insulin-like growth factor-1 (IGF-1) levels of school-age children.Methods From January 2016 to April 2017,124 children with Hp infection treated in Shaoxing People’s Hospital for the first time were selected as observation group,100 healthy children receiving physical examination were selected as control group. The children in observation group were treated by anti-Hp infection treatment. The distribution of clinical symptoms was recorded. Chest circumference,body weight,and height of children in the two groups were recorded. The levels of serum zinc (Zn),hemoglobin,iron (Fe),blood urea nitrogen,albumin,RPB,SF,and IGF-1 were detected. According to treatment outcome,the children in observation group were divided into Hp radical treatment group (99 children) and Hp recurrence group (25 children),nutritional indicators,growth and development status before treatment and at two years after treatment in the two groups were observed,the correlation between serum IGF-1 level and BMI was analyzed.Results Among 124 children,abdominal distension was observed in 37 children,abdominal pain was observed in 111 children,poor sleep was observed in 41 children,sour regurgitation was observed in 40 children,anorexia was observed in 85 children,after treatment,these symptoms disappeared. Body weight,height,serum Zn,hemoglobin,Fe,blood urea nitrogen,albumin,RPB,and SF levels in observation group were statistically significantly lower than those in control group,while IGF-1 level in observation group was statistically significantly higher than that in control group (P<0. 05). After two years,body weight,height,serum Zn,blood urea nitrogen,albumin,RPB,and SF levels in Hp radical treatment group were statistically significantly higher than those in Hp recurrence group,while IGF-1 level in Hp radical treatment group was statistically significantly lower than that in Hp recurrence group (P<0. 05). In observation group,serum IGF-1 level was positively correlated with BMI (r = 0. 441,P<0. 05). Conclusion Hp infection has impact on growth and development,serum nutritional indicators of children,timely Hp radical treatment can significantly improve nutritional status,growth and development of children.
