摘要
目的调查首发精神分裂症患者社会化住院影响因素。方法采用一般资料调查表、精神分裂症患者“社会化住院”评估标准、医院焦虑抑郁量表、简易精神状态检查表、睡眠障碍评定量表、改良Barthel指数量表对612例首发精神分裂症患者进行调查,采用Logistic回归分析其社会化住院现状及影响因素。结果符合社会化住院标准首发精神分裂症患者157例(25.65%),影响社会化住院因素包括年龄、有无合并其他慢性病、婚姻状况、家庭人均月收入、医疗费支付方式、是否焦虑、有无认知障碍、睡眠障碍、日常生活活动能力水平(P<0.01)。结论首发精神分裂症患者社会化住院程度亟待降低,因此需要医院在医疗卫生部门的指导下,积极与社区服务中心、乡镇卫生院、社会福利院、社会慢性病康复中心、医养康养中心等社会性医护服务机构合作,形成精神分裂症患者出院后“医院-社区-家庭”三级延续性医护服务机制。医护人员对达到出院标准的首发精神分裂症患者进行积极心理引导和劝解,缓解其心理负担,消除其焦虑心理,帮助患者树立战胜疾病、重返社会的信心。
Objective To investigate influencing factors of the first-episode schizophrenics'socialized hospitalization.Methods The general data Questionnaire,the"socialized hospitalization"assessment standard for schizophrenia,the Hospital Anxiety and Depression Scale,the MiniMental State Examination,the Sleep Dysfunction Rating Scale,and Modified Barthel Index Scale were used to investigate 612 patients with first-episode schizophrenia.Logistic regression was used to analyze the status quo of socialized hospitalization and its influencing factors.Results There were 157 patients(25.65%)who met the criteria for socialized hospitalization with first-episode schizophrenia.The factors influencing socialized hospitalization included age,whether there were other chronic diseases,marital status,per capita monthly income of the family,payment method of medical expenses,anxiety,whether there were cognitive disorders,sleep disorders,and activities of daily living(P<0.01).Conclusion It is necessary to reduce the degree of socialization of first-episode schizophrenia patients in hospital need,therefore,the hospital under the guidance of medical and health departments,actively cooperate with community service center,townships health center,social welfare,social chronic disease rehabilitation center and social care services such as health care facilities institutions,form"hospital-community-family continuity"three-level medical service mechanism for patients with schizophrenia after discharge.Medical staff provide positive psychological guidance and counseling to first-episode schizophrenia patients who meet discharge standards,relieve their psychological burden,eliminate their anxiety,and help patients establish confidence in conquering the disease and reentering the society.
