摘要
目的评价综合护理干预促进乳腺癌患者术后患肢功能康复的临床效果。方法纳入2017年9月至2018年9月在陆军军医大学第一附属医院乳腺甲状腺外科手术的乳腺癌术后患者共336例进行回顾性研究,在同一时间选取A病区(干预组)和B病区(对照组)的患者各168例。年龄、婚姻状况、从事体力劳动和居住情况比较采用χ^(2)检验,教育程度、BMI、肿瘤临床分期、腋窝淋巴结清扫和患者对护理工作的满意度比较采用Mann-Whitney U检验,术前肩关节活动度评价采用t检验,术后2个月比较采用协方差分析。结果干预组和对照组乳腺癌患者术后2个月患肢水平前屈、水平后伸、外展、前屈、后伸、内旋、外旋和爬墙高度比较,差异均有统计学意义(F=894.621、655.310、8025.193、10779.431、1957.617、3525.204、2412.949、7.629,P均<0.001)。干预组护理满意度问卷调查的满意率为99.4%(167/168),高于对照组满意率88.7%(149/168),差异有统计学意义(Z=-3.808,P<0.001)。结论临床中针对性的实施综合护理干预,可明显提高乳腺癌患者术后患侧肩关节活动度,且患者对综合护理干预的满意率更高,值得临床推广应用。
Objective To evaluate the clinical effect of comprehensive nursing intervention to promote postoperative functional rehabilitation of affected limbs in patients with breast cancer.Methods A total of 336 patients with breast cancer in the Department of Breast and Thyroid Surgery,First Affiliated Hospital of Army Medical University from September 2017 to September 2018 were involved in this retrospective study,including 168 patients from Unit A(intervention group)and 168 patients from Unit B(control group).The comparison of age,marital status,physical working and housing status was performed byχ^(2) test.The comparison of education,BMI,clinical stage of tumor,axillary lymph node dissection and patient satisfaction with nursing were performed by the Mann-Whitney U test.The evaluation results on motion range of the shoulder joints before surgery were compared by t test and the results at 2 months after operation were compared by analysis of covariance.Results At 2 months after operation,there were significant differences in the degree of horizontal flexion,horizontal extension,abduction,anterior flexion,posterior extension,internal rotation,external rotation and wall climbing height between intervention group and control group(F=894.621,655.310,8025.193,10779.431,1957.617,3525.204,2412.949,7.629,all P<0.001).The satisfaction rate in the nursing satisfaction survey was 99.4%(167/168)in intervention group,significantly higher than 88.7%(149/168)in control group(Z=-3.808,P<0.001).Conclusion Appropriate and targeted comprehensive nursing interventions should be given to breast cancer patients after operation,which can significantly improve the motion range of the affected shoulder joints and increase patient satisfaction with nursing,worthy of clinical application.
