摘要
目的分析妇科子宫全切术手术室护理中加速康复外科(ERAS)理念的应用价值。方法纳入广州市中西医结合医院妇科2017年9月至2019年11月收治的需行子宫全切术患者94例,随机分为ERAS组与对照组,每组各47例,对照组接受常规手术室护理,ERAS组则基于ERAS理念实施手术室护理,比较两组患者干预前后焦虑(SAS)、抑郁(SDS)情况,记录术后皮下气肿、疼痛等并发症发生情况,记录患者住院时间、住院治疗总费用、首次下床活动时间,调查统计患者护理满意度情况。结果两组干预前SAS及SDS评分比较,差异无统计学意义(P>0.05),ERAS组患者干预后SAS、SDS评分较对照组显著降低,ERAS组术后并发症总发生率显著更低,住院首次下床活动时间及住院时间均更短,且住院治疗总费用显著更低,ERAS组总满意度显著高于对照组,差异均有统计学意义(P<0.05)。结论将ERAS理念用于妇科子宫全切术患者手术室护理中有利于改善患者围术期不良情绪,减少术后并发症,提高患者护理服务满意度并加速其康复。
Objective To analyze the application value of the concept of enhanced recovery after surgery(ERAS)in the nursing of operating room in gynecological hysterectomy.Methods A total of 94 patients who needed hysterectomy admitted to department of gynecology,Guangzhou Hospital of Integrated Traditional and West Medicine from September 2017 to November 2019 were randomly divided into the ERAS group(n=47)and the control group(n=47).The control group was treated with conventional nursing of operating room,while the ERAS group was treated with nursing of operating room based on ERAS concept.The anxiety and depression of the two groups of patients were compared before and after intervention,and the incidence of complications such as subcutaneous emphysema and pain after operation were recorded.The hospitalization time,total hospitalization expenses and the first off-bed activity time of patients were recorded,and the nursing satisfaction of patients was investigated.Results There were no statistically significant differences in SAS and SDS scores between the two groups before intervention(P>0.05).After intervention,the scores of SAS and SDS in the ERAS group were significantly lower than those in the control group.The total incidence of complications in the ERAS group was significantly lower than that in the control group.The first off-bed activity time and hospitalization time in the ERAS group were both shorter than those in the control group,and the total hospitalization expenses were significantly lower.The total satisfaction of the ERAS group was significantly higher than that in the control group.All the above comparisons were with statistically significant differences(P<0.05).Conclusion The application of ERAS concept in the nursing of operating room in gynecological hysterectomy is beneficial to improve the patients’adverse emotions during perioperative period,reduce incidence of postoperative complications,improve patients’satisfaction with nursing services and accelerate their rehabilitation.
