肝郁化火证失眠症患者血清代谢特点分析 被引量:3

Analysis of the characteristics of serum metabolism in insomnia patients with liver depression transforming into fire syndrome

摘要 目的采用代谢组学技术研究失眠症肝郁化火证的血清代谢标志物,为失眠症肝郁化火证的证型判别提供代谢组学依据。方法设立肝郁化火证组(F组)、痰火内扰证组(G组)、阴虚火旺证组(H组)、健康组(A组)。根据失眠症及中医相关证型诊断标准筛选患者,每组10例,对照患者基线情况筛选10例健康受试者,取受试者血清送往深圳华大基因公司进行代谢全谱检测。采用代谢组学数据分析软件metaX进行偏最小二乘法判别法检验、差异倍数分析和组间t检验,筛选潜在代谢标志物。在此基础上二次筛选与失眠症肝郁化火证相关的代谢标志物,并在"人源代谢物数据库"(The Human Metabolome Database)中检索代谢物的生物信息。结果筛选到两个差异代谢物:Arginyl-Glycine和Phytosphingosine-1-P,其中Arginyl-Glycine在失眠症肝郁化火证组表现为下调趋势,Phytosphingosine-1-P表现为上调趋势。结论 Arginyl-Glycine和Phytosphingosine-1-P都参与调节中枢神经系统的睡眠-觉醒状态,影响能量代谢、情绪,而Phytosphingosine-1-P还与肝脏疾病关系密切。推测Phytosphingosine-1-P可能是区分失眠症肝郁化火证与痰火内扰证、阴虚火旺证的典型血清代谢标志物。检测失眠症患者血清中的Arginyl-Glycine和Phytosphingosine-1-P,可为失眠症肝郁化火证的证型判别提供实验依据。 Objective To study the serum metabolic markers of insomnia with liver depression transforming into fire syndrome by metabolomics technology, so as to provide metabolomics basis for its syndrome differentiation. Methods Set up liver depression transforming into fire syndrome group(Group F), internal disturbance of phlegm-heat syndrome group(group G), Yin deficiency with hyperactive fire syndrome group(group H), health group(group A). Patients were screened according to the diagnostic criteria of insomnia and related syndrome types in traditional Chinese medicine, 10 cases in each group, and 10 healthy subjects were screened according to the baseline condition of the patients. Serum was collected for metabolic full-spectrum detection by Shenzhen BGI co. LTD. Metabolomics data analysis software metaX was used to perform partial least square discriminant test, fold change analysis and t-test between groups to screen potential metabolic markers. On this basis, the metabolic markers related to the syndrome of liver depression transforming into fire of insomnia were screened twice, and the biological information of the metabolites was retrieved in "The Human Metabolome Database".Results Two different metabolites were screened out: Arginyl-Glycine and Phytosphingosine-1-P, among which Arginyl-Glycine showed a downward trend in the liver depression transforming into fire syndrome group, and the expression of Phytosphingosine-1-P was an upward trend. Conclusion Both Arginyl-Glycine and Phytosphingosine-1-P are involved in the regulation of the sleep-wake state of the central nervous system, affect energy metabolism, mood, and Phytosphingosine-1-P is closely related to liver disease. It is speculated that Phytosphingosine-1-P may be a typical metabolic marker to distinguish insomnia from syndrome of liver depression transforming into fire, syndrome of internal disturbance of phlegm-heat and syndrome of Yin deficiency with hyperactive fire. Detection of Arginyl-Glycine and Phytosphingosine-1-P in serum of patients with insomnia can provide experimental basis for syndrome differentiation of insomnia with liver depression transforming into fire syndrome.
