摘要
目的探讨去骨瓣减压术在高血压脑出血手术治疗中的应用效果。方法纳入2016年5月—2018年12月医院收治的100例高血压脑出血患者,随机分组。单纯开颅清除血肿术组选择单纯开颅清除血肿手术方案,开颅清除血肿手术联合去骨瓣减压术组选择开颅清除血肿手术联合去骨瓣减压手术。比较两组治疗效果。结果开颅清除血肿手术联合去骨瓣减压术组GCS评分值高于单纯开颅清除血肿术组,NHISS评分值低于单纯开颅清除血肿术组,ADL日常生活能力评分高于单纯开颅清除血肿术组,颅内压低于单纯开颅清除血肿术组,术后并发症低于单纯开颅清除血肿术组,P<0.05。结论高血压脑出血患者实施开颅清除血肿手术联合去骨瓣减压手术可获得较好效果,其实施效果肯定,可降低颅内压,改善患者神经功能,改善患者日常生活能力,并改善患者的意识状况和减少并发症的发生。
Objective To explore the effect of decompressive operation with boneless flap in the treatment of hypertensive cerebral hemorrhage.Methods 100 patients with hypertensive cerebral hemorrhage from May 2016 to December 2018 were randomly divided into two groups.In the group of simple craniotomy to remove hematoma,the scheme of simple craniotomy to remove hematoma was selected.In the group of craniotomy to remove hematoma combined with decompressive operation,the operation of craniotomy to remove hematoma combined with decompressive operation was selected.Compare results.Results The scores of GCS,NHISS,ADL,ICP and complications were higher in the craniotomy group than those in the craniotomy group(P<0.05).Conclusion The operation of removing hematoma by craniotomy combined with decompressive operation of bone flap can get better effect.The effect is positive.It can reduce intracranial pressure,improve patients'nerve function,improve patients'ability of daily life,improve patients'consciousness and reduce complications.
