摘要
目的:基于古今医案云平台,探讨中医治疗帕金森病(parkinson′s disease,PD)的用药规律。方法:以湖南省中医药研究院附属医院脑病科于2015年1月至2020年6月住院治疗PD的医案为数据来源,选取中医药治疗PD中药方剂。结果:选取180例PD患者临床方剂进行数据分析,患者年龄集中在60岁以上,比例为88.9%;中药方剂涉及中药226味,方剂中药物使用频次≥60次的前3位药物为白芍、当归、丹参;药物组合频次位于前3位的为"白芍-当归""白芍-丹参""枸杞子-白芍"。四气以温、平性药物为主,五味则以甘最多,归经以肝经频次最高。关联分析出现频次最高为白芍配伍当归;聚类配伍可将药物聚为6类,药物功效分析主要为平抑肝阳。结论:中医治疗PD多用温性、平性的药物,五味则以甘、辛最多,主归肝经,常以白芍、当归、丹参、枸杞子相互配伍,主要功效为平抑肝阳、养血柔肝,根据兼证不同,佐以滋补肝肾、潜镇熄风、益气健脾等。
Objective:To explore the medication rules of TCM in the treatment of Parkinson′s disease(PD)based on the ancient and modern medical record cloud platform.Methods:The clinical formulas of TCM for PD treatment were selected from the medical cases of the Department of Encephalopathy,Hunan Academy of Traditional Chinese Medicine Affiliated Hospital from January 2015 to June 2020.Results:A total of 180 cases of PD patients′clinical formulas were selected for data analysis.The patients were mostly over 60 years old,accounting for 88.9%.There were 226 kinds of Chinese medicinals involved.The top three ones used more than 60 times were Baishao(Radix Paeoniae Alba),Danggui(Radix Angelicae Sinensis)and Danshen(Radix Salviae Miltiorrhizae).The top three medicinal combinations were as follows:Baishao-Danggui,Baishao-Danshen,and Gouqizi(Fructus Lycii)-Baishao.In terms of four properties,the mild medicinals were mainly used.In terms of five flavors,the sweet medicinals were mainly used.In terms of meridian tropism,the frequency of liver meridian was the highest.Correlation analysis showed that the highest frequency combination was Danggui-Baishao.Cluster compatibility could be divided into six categories,and the main medicinal efficacy analysis was to suppress liver yang.Conclusion:In the treatment of PD,Chinese medicinals mostly use warm and mild ones,while the five flavors are mostly sweet and pungent.They mainly belong to the liver meridian.They are often combined with Baishao,Danggui,Danshen and Gouqizi.Their main functions are to suppress liver yang,nourish blood and soften liver.According to different concurrent syndromes,they are supplemented with nourishing liver and kidney,suppressing wind and invigorating qi and activating spleen.
