摘要
目的分析慢性阻塞性肺疾病(COPD)患者营养不良与全身炎症反应关系。方法回顾性选取2018年1月至2020年6月本院COPD患者220例,依据营养状况分为2组,一组营养不良组[体质量指数(BMI)<18.5 kg/m2,110例],一组营养正常组(BMI≥18.5 kg/m2,110例),统计分析2组患者的血清炎症因子水平,分析营养不良组患者的各营养指标及其与全身炎症反应关系。结果营养不良组患者的血清C反应蛋白(CRP)、中性粒细胞百分率(NEUT)、平均血小板体积(MPV)水平均显著高于营养正常组(P<0.05)。营养不良组患者的BMI、理想体质量百分比(IBW)、上臂围(MAC)、三头肌皮皱厚度(TSF)、血清前白蛋白(PA)、白蛋白(ALB)水平分别为(20.6±3.9)kg/m2、(100.5±10.5)%、(22.5±3.7)cm、(8.6±1.0)mm、(201.4±39.0)mg/L、(41.9±5.9)g/L。CRP、NEUT、MPV与BMI、IBW、PA、ALB均呈负相关关系(P<0.05),但与MAC、TSF均无相关性(P>0.05)。结论COPD患者营养不良与全身炎症反应关系密切。
Objective To analyze the relationship between malnutrition and systemic inflammatory response in COPD patients.Methods Two hundred and twenty cases of patients with COPD in our hospital from January 2018 to June 2020 were retrospectively selected and divided into two groups according to their nutritional status,one group was malnutrition group(BMI<18.5kg/m2,n=110),the other group was normal nutrition group(BMI≥18.5 kg/m2,n=110).The levels of serum inflammatory factors in the two groups were statistically analyzed,and the relationship between nutritional indexes and systemic inflammatory response in both groups were analyzed.Results The levels of TNF-α,IL-1βand MPV in the malnutrition group were significantly higher than those of the normal group(P<0.05).The levels of BMI,IBW,MAC,TSF,PA and ALB in the malnutrition group were(20.6±3.9)kg/m2,(100.5±10.5)%,(22.5±3.7)cm,(8.6±1.0)mm,(201.4±39.0)mg/L,(41.9±5.9)g/L,respectively.The TNF-α,IL-1βand MPV were negatively correlated with BMI,IBW,PA and ALB(P<0.05),but not with MAC and TSF(P>0.05).Conclusion Malnutrition is closely related to systemic inflammation in COPD patients.
