摘要
目的探讨微创针刀治疗手指近指间关节掌板损伤修复术后关节屈曲畸形的疗效.方法选取2014年5月至2018年3月余姚市中医医院收治的手指近指间关节掌板损伤修复术后掌板挛缩导致近指间关节屈曲畸形的患者32例,按照随机数表法随机分为观察组和对照组,每组各16例(16指),对照组进行保守功能锻炼,观察组予微创针刀治疗,随访6个月~2年,评价治疗后关节活动及患者满意情况,比较两组的治疗效果.结果针刀治疗后,患者近指间关节得到明显松解,关节较松解前显著伸直,关节被迫屈曲位解除,术后7个月随访,患者近指间关节屈曲如常,未有粘连反复.观察组患者的疗效优良率(93.3%)明显高于对照组(56.3%).结论针刀治疗手指近指间关节掌板损伤修复术后的关节屈曲畸形安全有效,并具有保护局部血运、操作简易、恢复快等优点,值得临床推广.
Objective To explore the curative effect of minimally invasive needle-knife in the treatment of joint flexion deformity after repairing the finger near interphalangeal joint metac arpal injury.Methods Thirty-two patients with flexion deformity of the proximal interphalangeal joint after the repair of the palmar plate injury of the proximal interphalangeal joint of the finger who were admitted to Yuyao Hospital of Traditional Chinese Medicine from May 2014 to March 2018 were selected and randomly divided according to the random number table.The observation group and the control group each had 16 cases(16 fingers),the control group received conservative functional exercises,and the observation group was treated with minimally invasive needle knife.Followed up for 6 months to 2 years to evaluate joint activity and parient satisfaction after treatment,and compared the treatment effects of the two groups.Results After needle knife therapy,the patient's proximal interphalangeal joints were significantly loosened,the joints were significantly straightened compared to before the release,and the joints were forced to he flexed.Follow-up 7 months after the operation,the patient's proximal interphalangeal joints flexed as usual without repeated adhesions.The excellent and good rate of the observation group(93.3%)was significantly higher than that of the control group(56.3%).Conclusion Needle knife therapy is safe and effective for the treatment ot'joint flexion defonnities after the repair of the palmar plate injury of the finger near the interphalangeal joint,and has the advantages of protecting local blood supply,simple operation and quick recovery,and is worthy of clinical promotion.
