摘要
目的分析循环ciRs-126与脓毒症性急性肾损伤(sepsis induced acute kidney injury,S-AKI)的相关性.方法前瞻性纳入2016年11月—2020年2月江门市中心医院收治脓毒症患者92例,根据患者肾功能恶化程度将患者分为S-AKI组(n=43)及非S-AKI(n=49)组,同期选择50例非脓毒症患者归为对照组,所有患者均抽取静脉血应用RT-PCR法检测循环ciRs-126表达情况,通过Pearson相关性分析ciRs-126与患者临床指标的相关性,通过ROC曲线评价ciRs-126诊断S-AKI的临床价值.结果与非S-AKI及对照组相比,①S-AKI组SCr、BUN、APACHEⅡ评分、SOFA评分及ciRs-126水平均显著升高,但eGFR显著降低;②ciRs-126与SCr(r=0.261)、BUN(r=0.419)、APACHEⅡ评分(r=0.318)及SOFA评分(r=0.507)呈正相关关系,但与eGFR(r=-0.411)呈负相关关系;③ROC曲线可知,ciRs-126诊断S-AKI的曲线下面积(AUC)(95%CI)为0.87(0.80~0.94).结论S-AKI患者循环ciRs-126表达增加,与病情有一定相关性,是潜在诊断S-AKI的指标.
Objective To explore the correlation between circulating ciRs-126 and septic induced acute kidney injury(S-AKI).Methods 92 patients with sepsis in our hospital were prospectively included from November 2016 to February 2020.All of them were divided into S-AKI group(n=43)and non-S-AKI(a=49)group,meanwhile 50 non-sepsis parents were selected as control group at the same time.Venous blood of subjects were collected and detected the expression of circulating ciRs-126 by using RT-PCR,the correlation between ciRs-126 and clinical indicators were analyzed by using Pearson correlation analysis and the clinical value of ciRs-126 in diagnosing S-AKI were evaluated by using ROC curve.Results Compared with non-S-AKI and control group,SCr,BUN,APACHE Ⅱ score,SOFA score and ciRs-126 in S-AKI group were significantly increased,while eGFR was significantly reduced;ciRs-126 had a positive corre-lated with SCr(r=0.261),BUN(r=0.419),APACHE Ⅱ score(r=0.318)and SOFA score(r=0.507),while had a negatively correlated with eGFR(r=-0.411);The ROC curve shows that the area under the curve(AUC)(95%Cl)of ciRs-126 for diagnosis of S-AKI was 0.87(0.80~0.94).Conclusion The expression of circulating ciRs-126 in patients with S-AKI is increased,which is related to the condition of the disease and is a potential indicator for the diagnosis of S-AKI.
