摘要
目的:评价中药汤剂治疗急性胰腺炎系统评价的方法学质量和结局指标的可靠程度。方法:计算机检索PubMed、万方、维普、中国知网4个数据库,收集公开发表的关于中药汤剂治疗急性胰腺炎的系统评价及Meta分析。检索时限为建库至2019年2月1日。2名研究者分别独立进行文献筛选和资料提取,采用AMSTAR2量表评价纳入研究的方法学质量,同时运用GRADE系统对结局指标进行证据质量分级。结果:共纳入15篇系统评价,73个结局指标。所有系统评价均对原始研究进行了偏倚风险评估,9篇使用Cochrane risk of bias偏倚评估工具,5篇使用Jadad量表评分,1篇使用EBM。AMSTAR2评分显示:7篇为低质量,8篇为极低质量。GRADE评级显示:共有43个结局指标为极低级,10个为低级,17个为中等级,仅有3个结局指标证据级别为高级。结论:目前中药汤剂治疗急性胰腺炎系统评价的方法学质量属低水平,结局指标的可靠程度相对较低,期待未来开展更多科学而严谨的研究以期得到更加准确、科学、高质量的临床证据,从而为急性胰腺炎的治疗和管理提供指导。
Objective:To assess the reliability of methodological quality and outcome indicators of systematic review of herbal decoction in the treatment of acute pancreatitis(AP).Methods:PubMed,Wanfang,VIP and CNKI were searched to collect the published systematic reviews and meta-analysis on the treatment of acute pancreatitis with herbal decoction.The retrieval time is from database building to February 1,2019.Two researchers independently screened the literature and extracted data,AMSTAR2 scale was used to evaluate the methodological quality of the included studies,and GRADE system was used to grade the quality of evidence of the outcome indicators.Results:A total of 15 systematic reviews and 73 outcome indicators were included.All systematic reviews assessed the risk of bias in the original study,nine of them used Cochrane risk of bias assessment tool,five used Jadad scale score,and one used EBM.AMSTAR2 score showed that seven articles were of low quality and eight articles were of very low quality.GRADE rating showed that 43 outcome indicators were very low-grade,ten were low-grade,17 were medium grade,and only three were high-grade.Conclusion:At present,the methodological quality of systematic evaluation of herbal decoction in the treatment of acute pancreatitis is at a low level,and the reliability of outcome indicators is relatively low.It is expected to carry out more scientific and rigorous researches in the future in order to obtain more accurate,scientific and high-quality clinical evidence,so as to provide the guidance for the treatment and management of acute pancreatitis.
