嗜铬粒蛋白B在晚期神经内分泌肿瘤中的应用 被引量:1

摘要 目的评估嗜铬粒蛋白B(chromogranin B,CgB)在神经内分泌肿瘤(neuroendocrine tumor,NET)诊断中的作用。方法选取2018年5月至2019年10月中日友好医院收治的53例晚期NET患者纳入NET组,同期于本院健康体检者37例纳入对照组,采用酶联免疫吸附测定检测两组研究对象血清嗜铬粒蛋白A(chromogranin A,CgA)和CgB,并分析二者与NET临床组患者病理特征的关系,采用受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)评价血清CgA、CgB及二者联合诊断NET的效能。结果NET组患者血清CgA和CgB水平均显著高于对照组(均P<0.05)。CgB的曲线下面积(area under the curve,AUC)为0.84,临界值为633.10 pg/ml;CgA的AUC为0.71,临界值为8.06 ng/ml;二者联合诊断NET的能力并不高于单独测定CgB的诊断能力。同时存在肝及肝外转移、胰腺NET、CgA阳性的患者血清CgA水平显著高于肝转移、直肠NET、CgA阴性的患者(均P<0.05);病理分级G3、肝转移肿瘤负荷>50%的患者血清CgB水平显著高于病理分级G1/G2、肝转移肿瘤负荷≤50%的患者(均P<0.05)。结论相较于血清CgA,血清CgB有更好的诊断作用,可作为晚期胰腺和直肠NET的诊断标志物应用于临床。 Objective To evaluate the role of chromogranin B in the diagnosis of neuroendocrine tumor(NET).Method From May 2018 to October 2019,53 patients with advanced NET admitted to China-Japan Friendship Hospital were included in NET group,while 37 healthy people in the same hospital were included in control group.Serum CgB and CgA were detected in two groups by enzyme-linked immunosorbent assay,and the relationship between the two biomarkers and clinical characteristics were analyzed.Receiver operating characteristic curve(ROC curve)was used to evaluate the efficacy of serum CgA,CgB and their combined diagnosis of NET.Result The levels of serum CgB and CgA in NET group were significantly higher than those in control group(all P<0.05).The area under the curve(AUC)of CgB was 0.84,and the cut-off value was 633.10 pg/ml;the AUC of CgA was 0.71,and the cut-off value was 8.06 ng/ml.The diagnostic ability of CgA combined with CgB was not higher than that of CgB alone.The serum CgA level of patients with liver and extrahepatic metastasis,tumor originating from pancreas and positive CgA were significantly higher than those of patients with liver metastasis,tumor originating from rectum and negative CgA(all P<0.05);the serum CgB levels of patients with pathological grade G3 and liver metastasis tumor burden>50%were significantly higher than those of patients with pathological grade G1/G2 and liver metastasis tumor load≤50%(all P<0.05).Conclusion Compared with serum CgA,serum CgB has a better diagnostic effect and may be used as a biomarker of pancreatic and rectal NET.
