摘要
目的比较正骨手法结合中药辨证治疗与内固定手术治疗三踝骨折的疗效。方法68例三踝骨折患者,随机分为研究组和对照组,每组34例。研究组采用正骨手法夹板外固定结合中药辨证治疗,对照组采用切开复位内固定手术治疗。对比两组患者的住院时间、骨折愈合时间、下地时间、Kofoed踝关节功能评分、治疗效果、并发症发生情况、生活质量评分。结果研究组患者下地时间晚于对照组,住院时间、骨折愈合时间均短于对照组,差异具有统计学意义(P<0.05)。治疗后,研究组患者疼痛、功能以及活动度评分均高于对照组,差异具有统计学意义(P<0.05)。研究组患者的治疗总有效率为94.12%,高于对照组的76.47%,差异具有统计学意义(P<0.05)。研究组患者的并发症发生率为2.94%,低于对照组的17.65%,差异具有统计学意义(P<0.05)。研究组患者的心理健康、躯体健康、社会功能以及日常生活评分均高于对照组,差异具有统计学意义(P<0.05)。结论对三踝骨折患者实施正骨手法结合中药辨证治疗具有显著效果,能有效减轻患者的疼痛感,促进踝关节功能恢复,进而提高生活质量,且该治疗的并发症较少,是一种具有较高安全性、有效性的治疗方式。
Objective To compare the efficacy of bone setting manipulation combined with traditional Chinese medicine syndrome differentiation and internal fixation for the treatment of three-ankle fractures.Methods A total of 68 patients with three-ankle fractures were randomly divided into research group and control group,with 34 cases in each group.The research group was treated with bone setting and splint external fixation and traditional Chinese medicine syndrome differentiation,and the control group was treated with open reduction and internal fixation.The hospitalization time,fracture healing time,off-bed time,Kofoed ankle function score,therapeutic effect,occurrence of complications and quality of life score were compared between the two groups.Results The off-bed time of the research group was later than that of the control group,and hospitalization time and fracture healing time were shorter than those of the control group.All the difference was statistically significant(P<0.05).After treatment,the pain,function and motor score of the research group were higher than those of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the research group was 94.12%,which was higher than 76.47%of the control group,and the difference was statistically significant(P<0.05).The incidence of complications of the research group was 2.94%,which was lower than 17.65%of the control group,and the difference was statistically significant(P<0.05).The mental health,physical health,social function and daily life scores of the research group were all higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of bone setting manipulation and traditional Chinese medicine syndrome differentiation shows significant effect on patients with three ankle fractures,which can effectively reduce the pain,promote the recovery of ankle joint function,and improve the quality of life of the patient.It is a safe and effective treatment with fewer complications.
