摘要
目的了解并比较入住不同类型医养结合机构的老年人对养老服务的满意度,分析影响因素并提出针对性建议。方法采取方便抽样的方法,对入住大同市和太原市6家医养结合机构的436名老年人进行问卷调查。结果满意度总得分均值为(7.03±0.79),得分最高的3个指标依次为生活照料服务(8.00±0.75)、出入院服务(7.62±1.13)、安全照护服务(7.58±0.81),得分最低的3个指标依次为既定服务下对价格的评价(5.87±1.31)、心理精神支持服务(5.47±1.16)、安宁服务(4.57±0.96);不同类型医养结合机构在生活照料服务、医疗护理服务、康复服务、既定服务下对价格的评价、对机构的支持程度方面的差异有统计学意义(P<0.05);性别、文化程度、家庭人均月收入、患慢性病数量、子女数、入住机构的时间、每月支付费用对满意度有显著影响(P<0.05)。结论医养结合机构健康养老服务得到认可,但存在心理和精神支持不足、安宁服务匮乏等问题;不同类型医养结合机构服务各有优劣,医疗资源和养老资源的一体化机制建设是关键,同时应建立多元投入的长期护理保险制度,并兼顾商业性和普惠性。
Objective To investigate and compare the service satisfaction of the elderly living in medical and nursing care institutions,to further analyze the main influencing factors and to give some specific suggestions.Methods Convenience sampling was used to enroll a total of 436 senior citizens from 6 medical and nursing care institutions in Datong City and Taiyuan City for a questionnaire.Results The grand average was(7.03±0.79).The top three service items were daily life care(8.00±0.75),hospitalization and discharging service(7.62±1.13)and safe attendance(7.58±0.81).The last three service items were evaluation of price(5.87±1.31),emotional support and psychological service(5.47±1.16)and hospice service(4.57±0.96).Scores for daily life care,medical treatment and nursing service,rehabilitation service,evaluation of price and level of support of the elderly were significantly different among different integrated medical and nursing care institutions(P<0.05).The satisfaction was greatly influenced by gender,educational background,average household income per month,number of chronic diseases,number of children,length of living in institution and monthly cost(P<0.05).Conclusions The service of integrated medical and nursing care institutions was approved.But there were still such problems as insufficient emotional and psychological support and hospice service.Great attention should be paid to inadequate emotional support and psychological service and lacking of hospice service.Integration of medical resources and old-aged supporting resource is the key.Building long-term care insurance system should consider both the commercial and public interests.
