摘要
目的分析阿莫西林引起不良反应的药学情况。方法39例采用阿莫西林治疗后出现不良反应且已上报不良反应监测部门的患者为研究对象,分析患者的用药及出现的不良反应情况,探讨阿莫西林引发不良反应的原因及预防措施。结果39例患者中,用药时间<7 d患者的占比为76.9%,大于用药时间≥7 d患者的23.1%;给药途径及剂量为口服,0.5 g/次,3次/d患者的占比为89.7%,大于给药途径及剂量为静脉滴注,0.3 g/次,1次/d患者的10.3%。用药30 min内出现不良反应的占比为10.3%(4/39),用药30 min~1 d出现不良反应的占比为23.1%(9/39),用药2~7 d出现不良反应的占比为46.2%(18/39),用药8~14 d出现不良反应的占比为12.8%(5/39),用药14 d后出现不良反应的占比为7.7%(3/39)。不良反应包括消化系统反应(30.8%),血液系统反应(5.1%),过敏反应(46.2%),神经系统反应(12.8%),肾功能损害(5.1%)。治疗有效率为92.3%(36/39);轻度不良反应患者,给予停药处理,不良反应自行消失;重度不良反应患者,给予停药后对症治疗,经积极干预后,不良反应消失。结论阿莫西林用药引发的不良反应涉及消化系统反应、血液系统反应、过敏反应、神经系统反应、肾功能损害,需引起临床的重视。
Objective To analyze the pharmaceutical situation of adverse reactions caused by amoxicillin.Methods 39 patients with adverse reactions after amoxicillin treatment and reported to the adverse reaction monitoring department were selected as the research subjects.The medication and adverse reactions were analyzed,the causes and preventive measures of the adverse reactions caused by amoxicillin were discussed.Results Among 39 patients,76.9%of patients took medication for<7 d,which was greater than 23.1%of patients took medication for≥7 d;The proportion of patients with oral administration,0.5 g/time,3 times/d,was 89.7%,which was higher than 10.3%of patients with intravenous drip,0.3 g/time,1 time/d.The proportion of adverse reactions occurring within 30 min of medication was 10.3%(4/39),the proportion of adverse reactions occurring within 30 min to 1 d of medication was 23.1%(9/39),the proportion of adverse reactions occurring within 2-7 d of medication was 46.2%(18/39),the proportion of adverse reactions occurring 8-14 d after medication was 12.8%(5/39),and the proportion of adverse reactions occurring after 14 d of medication was 7.7%(3/39).Adverse reactions included digestive system reaction(30.8%),hematological system reaction(5.1%),allergic reaction(46.2%),nervous system reaction(12.8%),renal function damage(5.1%).The effective rate of treatment was 92.3%(36/39);patients with mild side effects should be treated with drug withdrawal,and the side effects disappeared on their own;patients with severe side effects were given symptomatic treatment after drug withdrawal,and the side effects disappeared after active intervention.Conclusion The adverse reactions caused by amoxicillin involve digestive system,blood system reaction,allergic reaction,nervous system reaction and renal function damage,which should be paid attention to in clinic.
