高频超声引导下乳腺细针定位在乳腺微小肿物切除中的应用 被引量:4

摘要 目的:研究高频超声引导下乳腺细针定位在乳腺微小肿物切除中的应用效果。方法:选取医院收治并完成随访的60例乳腺微小肿物患者,依据治疗方法的不同将其分为观察组和对照组,每组30例。观察组行高频超声引导下乳腺细针定位切除治疗,对照组行高频超声引导下体表定位切除治疗。所有患者术后均接受1年随访,比较两组手术相关指标(手术时间、术中出血量及住院时间),切除组织体积,复发情况(随访半年和1年的复发情况),以及并发症发生率。结果:观察组术中出血量为(21.45±3.87)ml,对照组为(23.13±4.13)ml,两组比较差异无统计学意义(t=1.626,P>0.05);观察组手术时间和住院时间分别为(16.44±1.97)min和(4.86±0.46)d,均短于对照组的(18.16±2.48)min和(5.32±0.53)d,两组比较差异有统计学意义(t=2.975,t=3.590;P<0.05);观察组切除组织体积(1.53±0.32)ml,小于对照组(1.85±0.35)ml,两组比较差异有统计学意义(t=3.696,P<0.05);观察组半年复发率为0.00%,与对照组的3.33%对比,差异无统计学意义(χ^(2)=1.404,P>0.05);观察组1年复发率为3.33%,低于对照组(26.67%),差异有统计学意义(χ^(2)=4.706,P<0.05);观察组并发症发生率为3.33%,与对照组的13.33%比较,差异无统计学意义(χ^(2)=0.873,P>0.05)。结论:高频超声引导下乳腺细针定位可有效辅助乳腺微小肿物切除手术,对微小肿块的检出与定位更为准确,并可缩短手术时间,加快患者术后恢复,且安全性较高,复发率较低。 Objective:To investigate the application effect of fine needle localization on breast with guidance of high-frequency ultrasound in the resection of micro breast masses.Methods:60 patients with micro breast masses who admitted to hospital and completed the follow-up were selected.And they were divided into observation group and control group according to the different treatment methods,with 30 cases in each group.The observation group underwent the resection treatment of fine needle localization on breast with guidance of high-frequency ultrasound,and the control group underwent the resection treatment of body surface location under the guidance of high-frequency ultrasound.All patients were followed up for 1 year after surgery,and the related indicators of surgery(operation time,intraoperative blood loss and length of hospital stay),the volume of resected tissues,recurrence(recurrence after half a year and one year of follow-up)and incidence of complications between the two groups were compared.Results:There was no statistical significant difference in intraoperative blood loss between observation group(21.45±3.87)ml and control group(23.13±4.13)ml(t=1.626,P>0.05).The operation time(16.44±1.97 min)and length of stay(4.86±0.46 d)of observation group were significantly shorter than those[(18.16±2.48 min),(5.32±0.53 d)]of control group(t=2.975,t=3.590,P<0.05),respectively.The volume of resected tissues(1.53±0.32 ml)of observation group was significantly lower than that(1.85±0.35 ml)of control group(t=3.696,P<0.05).There was no significant difference in the recurrence rate after half a year between observation group(0.00%)and control group(3.33%)(χ^(2)=1.404,P>0.05).And there was no significant difference in the recurrence rate after one year between observation group(3.33%)and control group(26.67%)(χ^(2)=4.706,P>0.05).In addition,there was no significant difference in the incidence of complications between observation group(3.33%)and control group(13.33%)(χ^(2)=0.873,P>0.05).Conclusion:The fine needle localization on breast with the guidance of high-frequency ultrasound can effectively assist to the resection of micro breast masses,and it is more accurate in the detection and localization of micro breast masses,and it can shorten the operation time,and accelerate the postoperative recovery of patients,and it has higher safety and lower recurrence rate.
