摘要
目的:探讨中医特色护理干预对帕金森病患者非运动症状的影响,以期为帕金森病患者的临床护理提供参考。方法:选取62例2017年5月-2019年5月于广东省中医院珠海医院脑病内科住院治疗的帕金森病伴非运动症状的患者,采用随机数字表法分为对照组和观察组,每组各31例。对照组进行帕金森病临床常规护理,观察组在常规护理的基础上增加中药穴位贴敷、耳穴压豆、中药热罨包以及穴位按摩等中医特色护理干预措施。护理前后分别对患者进行汉密顿抑郁量表(Hamilton Depression Scale,HAMD)、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)、蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)及匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)评分,观察比较两组患者焦虑或抑郁、认知障碍、睡眠障碍、尿潴留和便秘发生率及患者满意度情况。结果:与护理干预前相比,护理干预后两组患者的HAMD评分、HAMA评分、PSQI评分明显降低,MoCA评分明显升高,差异均有统计学意义(P<0.05);而与对照组相比,观察组患者HAMD评分、HAMA评分、PSQI评分、尿潴留和便秘发生率明显降低,MoCA评分亦明显升高,差异均有统计学意义(P<0.05)。另外,观察组患者治疗满意度96.77%(30/31)明显高于对照组77.42%(24/31),差异有统计学意义(P<0.05)。结论:中医特色护理干预可有效改善帕金森病患者非运动症状,提高患者对治疗的满意度,值得在临床加以推广。
Objective:To explore the effect of traditional Chinese medicine characteristic nursing intervention on non-motor symptoms in patients with Parkinson's disease,and to provide reference for the clinical nursing of patients with Parkinson's disease.Methods:62 patients with Parkinson's disease and non-motor symptoms who were hospitalized in Zhuhai Hospital,Guangdong Hospital of Traditional Chinese Medicine from May 2017 to May 2019 were enrolled.They were divided into the control group and the observation group by using the random number table,with 31 cases in each group.The control group received the clinical routine nursing of Parkinson's disease.On the basis of routine nursing,the observation group was given acupoint application of traditional Chinese medicine,auricular pressure,traditional Chinese medicine hot pack and acupoint massage.And then patients were scored with the hamilton depression scale(HAMD),hamilton anxiety scale(HAMA),Montreal cognitive assessment(MoCA),and pittsburgh sleep quality index(PSQI)before and after treatment.The anxiety and depression,cognitive impairment,sleep disturbance,urinary retention and constipation incidence and patient satisfaction were compared between the two groups.Results:Compared with that before the nursing intervention,the HAMD score,HAMA score and PSQI score of the two groups were significantly lower after the nursing intervention,and the MoCA score was significantly increased,the difference was statistically significant(P<0.05).Compared with the control group,the HAMD score,HAMA score,PSQI score,urinary retention and constipation incidence were significantly lower in the observation group,and the MoCA score was significantly higher,the difference was statistically significant(P<0.05).In addition,the treatment satisfaction of the observation group(96.77%,30/31)was significantly higher than that of the control group(77.42%,24/31),and the difference was statistically significant(P<0.05).Conclusion:The characteristic nursing intervention of traditional Chinese medicine can effectively relieve the non-motor symptoms of patients with Parkinson's disease and improve patient satisfaction with treatment.It is worthy of promotion in clinical practice.
