摘要
目的探究小切口甲状腺切除术治疗甲状腺腺瘤的效果及对患者甲状腺激素的影响。方法86例甲状腺腺瘤患者,根据随机数字表法将其分为对照组及观察组,各43例。对照组采用传统开放手术治疗,观察组采用小切口甲状腺切除术治疗。对比两组患者的手术情况(手术时间、切口长度、术中出血量、瘢痕长度、住院时间),治疗效果,甲状腺激素[促甲状腺激素(TSH)、甲状腺素(T_(4))、三碘甲状腺原氨酸(T_(3))]水平,术后并发症发生情况。结果观察组患者的手术时间、切口长度、瘢痕长度、住院时间均短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。观察组患者的治疗总有效率93.02%高于对照组的76.74%,差异具有统计学意义(P<0.05)。观察组患者的TSH(2.08±1.02)mIU/L、T_(4)(134.52±40.36)nmol/L、T_(3)(1.98±0.62)nmol/L均低于对照组的(3.56±1.98)mIU/L、(169.62±59.64)nmol/L、(3.47±2.10)nmol/L,差异具有统计学意义(P<0.05)。观察组患者的并发症发生率6.98%低于对照组的23.26%,差异具有统计学意义(P<0.05)。结论小切口甲状腺切除术治疗甲状腺腺瘤的效果良好,可减少患者机体损伤,改善甲状腺激素水平,安全性高,值得应用。
Objective To investigate the effect of small incision thyroidectomy on thyroid adenoma and its influence on thyroid hormone.Methods A total of 86 patients with thyroid adenoma were divided into control group and observation group according to random numerical table,with 43 cases in each group.The control group was treated with traditional open surgery,and the observation group was treated with small incision thyroidectomy.The surgical situations(operation time,incision length,intraoperative blood loss,scar length,hospitalization time),therapeutic effect,thyroid hormone[hyroid-stimulating hormone(TSH),thyroxine(T_(4)),triiodothyronine(T_(3))]levels,and occurrence of postoperative complications were compared between the two groups.Results The operation time,incision length,scar length,hospitalization time of the observation group were shorter than those of the control group,and intraoperative blood loss was less than that of the control group.All the difference was statistically significant(P<0.05).The total effective rate 93.02% of the observation group was higher than 76.74%of the control group,and the difference was statistically significant(P<0.05).The TSH(2.08±1.02)mIU/L,T_(4)(134.52±40.36 nmol/L and T_(3)(1.98±0.62)nmol/L of the observation group were lower than 3.56±1.98)mIU/L,(169.62±59.64)nmol/L and(3.47±2.10)nmol/L of the control group,and the difference was statistically significan(P<0.05).The incidence of complications 6.98% of the observation group was lower than 23.26%of the control group,and the difference was statistically significant(P<0.05).Conclusion Small incision thyroidectomy has good effect in the treatment of thyroid adenoma,and it can reduce the body damage,improve the level of thyroid hormone with high safety,which is worthy of application.
