摘要
目的观察温阳生肌膏治疗慢性皮肤溃疡的临床疗效。方法将100例慢性皮肤溃疡患者采用随机数字表法分为治疗组和对照组,各50例。2组均给予常规基础治疗[包括注意休息,寻找并去除可能的病因(如控制血糖、抗凝等),抗感染,创面常规消毒与清创],在此基础上,2组分别予温阳生肌膏及重组牛碱性成纤维细胞生长因子凝胶(商品名为贝复新)均匀涂抹于创面,以无菌纱布包扎,每日换药1次,2组均以7 d为1个疗程,4个疗程后比较2组的临床疗效、创面愈合率、复发率、不良反应发生情况,以及治疗前后的溃疡皮损面积、溃疡色泽评分、新生肉芽组织评分。结果对照组总有效率为78%(39/50),治疗组为96%(48/50),2组比较差异有统计学意义(P<0.05);对照组创面愈合率为30%(15/50),治疗组为76%(38/50),2组比较差异有统计学意义(P<0.01);经过随访,治疗组复发率为0,对照组为8%(4/50),2组比较差异有统计学意义(P<0.05);治疗后,2组溃疡皮损面积明显缩小,溃疡色泽评分明显降低,新生肉芽组织评分明显升高,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组作用明显优于对照组(P<0.05);治疗过程中2组患者均未出现明显不良反应。结论温阳生肌膏治疗慢性皮肤溃疡疗效显著,可以促进创面肉芽组织的良好生长而使创面愈合,能明显提高创面愈合率、降低复发率,且无明显不良反应,值得临床推广应用。
Objective To observe the clinical efficacy of Wenyang Shengji Gao(温阳生肌膏)in the treatment of chronic skin ulcer.Methods Totally 100 cases with chronic skin ulcer were divided into treatment group and control group according to random number table method,with 50 cases in each group.The 2 groups were given routine basic treatment[including attention to rest,looking for and removing possible causes(such as blood sugar control,anticoagulation,etc.),anti infection,routine disinfection and debridement of wound surface].On the basis of these,the 2 groups were respectively treated with Wenyang Shengji Gao and recombinant bovine basic fibroblast growth factor gel(trade name Beifuxin),evenly smeared on the wound surface,bandaging with a sterile gauze pad and changing the dressing once a day.Seven days constituted 1 course of treatment for both groups.After 4 courses of treatment,the clinical efficacy,wound healing rate,recurrence rate,incidence of adverse reactions,ulcer skin lesion area,ulcer color and lustre score and neoformative granulation tissue score before and after treatment were compared between the 2 groups.Results The total effective rate was 78%(39/50)in the control group and 96%(48/50)in the treatment group,and the difference was statistically significant between the 2 groups(P<0.05).The wound healing rate was 30%(15/50)in the control group and 76%(38/50)in the treatment group,and the difference was statistically significant between the 2 groups(P<0.01).After follow-up,the recurrence rate was 0 in the treatment group and 8%(4/50)in the control group,and the difference was statistically significant between the 2 groups(P<0.05).After treatment,the area of ulcer skin lesions in the 2 groups significantly reduced,the ulcer color and lustre score significantly reduced,and the score of neoformative granulation tissue significantly increased,and the difference was statistically significant compared with the same group before treatment(P<0.05),and the effect of the treatment group was significantly better than that of the control group(P<0.05).No significant adverse reactions occurred in the 2 groups during the treatment.Conclusion Wenyang Shengji Gao has a significant efficacy on chronic skin ulcer,which can promote the good growth of wound surface granulation tissue and make the wound healing.It can markedly improve the healing rate and reduce the recurrence rate of the wound surface without marked adverse reactions,which is worth clinical promotion and application.
