摘要
目的比较多裂肌入路经椎间孔腰椎椎体间融合术(Transforaminal lumbar interbody fusion,TLIF)与后路腰椎椎体间融合术(Posterior lumbar interbody fusion,PLIF)治疗腰椎疾病的临床疗效。方法回顾性分析自2015-03—2017-06诊治的121例腰椎疾病,61例采用多裂肌入路TLIF术治疗(TLIF组),60例采用PLIF术治疗(PLIF组)。比较2组手术时间、术中出血量及术后1、2、6周疼痛VAS评分,比较2组术后6周椎间融合数。结果 121例均顺利完成手术并获得完整随访,随访时间3~38个月,平均18.5个月。TLIF组手术时间、术中出血量较PLIF组少,差异有统计学意义(P<0.05)。术后1周及术后2周TLIF组疼痛VAS评分低于PLIF组,差异有统计学意义(P<0.05);术后6周2组疼痛VAS评分、椎间融合数比较差异无统计学意义(P>0.05)。结论多裂肌入路TLIF治疗腰椎疾病具有手术创伤小、手术时间短、术中出血少等优点,具有一定临床推广价值。
Objective To compare the clinical effect between transforaminal lumbar interbody fusion(TLIF) through multifidus approach and posterior lumbar interbody fusion(PLIF) for lumbar diseases.Methods Retrospective analysis was performed on121 cases of lumbar diseases diagnosed and treated from March 2015 to June 2017. Sixty-one cases were treated with multifidus approach TLIF(TLIF group), and 60 cases with PLIF(PLIF group). Compare the operation time, intraoperative blood loss and pain VAS scores of the two groups at 1, 2, and 6 weeks after surgery, and compare the number of intervertebral fusions in the two groups at 6 weeks after surgery.ResultsAll 121 cases successfully completed the operation and received complete follow-up. The follow-up time was 3 to 38 months, with an average of 18.5 months. The operation time and intraoperative blood loss in the TLIF group were less than those in the PLIF group, and the difference was statistically significant(P<0.05). The pain VAS score of the TLIF group was lower than that of the PLIF group at 1 week and 2 weeks after the operation, the difference was statistically significant(P<0.05);the difference in the pain VAS score at 6 weeks after the operation between the two groups was not statistically significant(P>0.05). There was no significant difference in the number of intervertebral fusion at 6 weeks after operation between the two groups(P>0.05).ConclusionMultifidus approach TLIF in the treatment of lumbar diseases has the advantages of small trauma, short operation time and less intraoperative bleeding, and has great clinical promotion value.
