摘要
目的:分析导致接受手术后的肾盂及输尿管结石患者发生泌尿系统感染的危险因素。方法:回顾分析东莞市东华医院2018年1月至2019年5月收治的120例肾盂及输尿管结石患者的临床资料。对这些患者均进行手术治疗。根据术后是否发生泌尿系统感染将其分为未感染组(未发生泌尿系统感染,n=100)和感染组(发生泌尿系统感染,n=20)。对比两组患者的临床资料,从中找出导致感染组患者发生泌尿系统感染的危险因素。结果:两组患者血清降钙素原(Procalcitonin,PCT)、C-反应蛋白(C-reactive pr otein,CRP)、脂肪酶(Lipase,LPS)的水平相比,P<0.05。进行Logistic回归分析的结果提示,年龄>60岁、留置尿管的时间>7 d、使用抗菌药物的种类≥2种、合并有糖尿病、免疫功能差是导致感染组患者发生术后泌尿系统感染的独立危险因素(OR值:2.273、2.484、2.175、2.361、2.430,P<0.05)。结论:年龄>60岁、留置尿管的时间>7 d、使用抗菌药物的种类≥2种、合并有糖尿病、免疫功能差是导致接受手术后的肾盂及输尿管结石患者发生泌尿系统感染的独立危险因素。此类患者血清PCT、CRP、LPS的水平与其发生泌尿系统感染的风险密切相关。
Objective:To analyze the risk factors for urinary tract infection in patients with renal pelvis and ureteral calculi after surgery.METHODS:Clinical data of 120 patients with renal pelvis and ureteral calculi admitted to Dongguan Donghua Hospital from January 2018 to May 2019 were retrospectively analyzed.Surgical treatment was performed on all of these patients.According to the occurrence of postoperative urinary system infection,the patients were divided into uninfected group(no urinary system infection occurred,n=100)and infected group(urinary system infection occurred,n=20).The clinical data of the two groups were compared to find out the risk factors of urinary system infection in the infection group.RESULTS:The levels of Procalcitonin(PCT),C-reactive protein(CRP)and Lipase(LPS)in serum of 2 groups were compared(P<0.05).Logistic regression analysis indicated that age>60 years old,>7 days of indwelling catheter,≥2 types of antibiotics,combined with diabetes and poor immune function were independent risk factors for postoperative urinary system infection in the infection group(OR value:2.273,2.484,2.175,2.361,2.430,P<0.05).CONCLUSIONS:The age of>60 years old,the duration of ureteral catheter indwelling>7 days,the use of antibiotics≥2 types,combined with diabetes and poor immune function are the independent risk factors for urinary system infection in patients with renal pelvis and ureteral calculi after surgery.The levels of serum PCT,CRP and LPS in these patients are closely related to the risk of urinary tract infection.
