摘要
目的回顾性分析某医院碳青霉烯类抗菌药物的应用情况,探讨其用药合理性。方法350例某医院应用碳青霉烯类抗菌药物的患者为研究对象,对患者的用药情况进行比较分析,并对用药合理性进行评估。结果注射用美罗培南的用药频度(DDDs)为448.75,药物利用指数(DUI)为0.41,表示实际使用的倾向性较小;注射用亚胺培南西司他丁钠的DDDs为1484,DUI为1.20,表示实际使用的倾向性较大,提示其用药可能存在不合理性;但实际使用是否合理仍需针对病例进行分析。350例患者中,用药合理298例,用药合理率为85.14%;用药不合理52例,用药不合理率为14.86%,包括:适应证不合理、用法用量不合理及没有进行病原微生物送检。结论某医院碳青霉烯类抗菌药物的临床应用存在一定的不合理性,必须采取进一步的改善措施,促进碳青霉烯类抗菌药物更加规范的使用。
Objective To retrospectively analyze the application of carbapenem antibiotics in a hospital,and explore the rationality of drug use.Methods 350 patients with carbapenems antibiotics in a hospital were selected as the research subjects.The medication situation of patients was compared and analyzed,and the rationality of drug use was evaluated.Results The drug daily dosages(DDDs)of meropenem for injection was 448.75,and the drug utilization index(DUI)was 0.41,indicating that the tendency of actual use was low.The DDDs of imipenem and cilastatin sodium for injection was 1484,and the DUI was 1.20,indicating that the tendency of actual use was greater,suggesting that its medication may be unreasonable;however,whether the actual use was reasonable still needs to be analyzed for the case.Among 350 patients,298 cases were rational drug use,with a reasonable rate of 85.14%,52 cases of irrational drug use,with a reasonable rate of 14.86%,including unreasonable indications,unreasonable usage and dosage,and failure to submit pathogenic microorganisms for inspection.Conclusion The clinical application of carbapenem antibiotics in the hospital is not reasonable,and further improvement measures should be taken to promote more standard use of carbapenem antibiotics.
