摘要
目的探讨慢性萎缩性胃炎(CAG)患者抗Hp治疗后内镜特征及胃黏膜组织中微小RNA-1296-5p(miR-1296-5p)和微小RNA-155-5p(miR-155-5p)表达水平变化.方法选取2019年4月-2020年4月浙江中医药大学附属第二医院114例CAG合并幽门螺杆菌(Hp)感染患者为研究对象,观察抗Hp治疗后内镜特征变化,采用逆转录聚合酶链式扩增反应(RT-PCR)检测胃黏膜组织miR-1296-5p、miR-155-5p表达水平变化.结果CAG患者治疗后花斑样改变、血管显露、黏膜颗粒、肠化结节、黏膜糜烂和黏膜内出血内镜表现占比均显著低于治疗前(P<0.05);与治疗前比较,治疗后萎缩胃黏膜组织中miR-1296-5p、miR-155-5p表达水平显著升高(P<0.05);miR-1296-5p低表达胃黏膜萎缩、慢性炎症、肠化生中、重度患者占比均较高(P<0.05);miR-155-5p低表达患者慢性炎症中、重度患者占比较高(P<0.05);miR-1296-5p与miR-155-5p联合检测对CAG合并Hp感染患者不良预后的曲线下面积(AUC)为0.857,显著高于miR-1296-5p、miR-155-5p单独检测.结论CAG合并Hp感染患者经抗Hp治疗后可以改善胃部内镜表现,miR-1296-5p、miR-155-5p在萎缩胃黏膜组织均呈低表达状态并且两者联合可以提高不良预后预测效能.
OBJECTIVE To explore the endoscopic features and expression levels of micrRNA-1296-5 p(miR-1296-5 p)and micrRNA-155-5 p(miR-155-5 p)in gastric mucosal tissues of patients with chronic atrophic gastritis(CAG)after anti-Hp treatment.METHODS Totally 114 CAG patients complicated with Hp infection who were treated in the Second Affiliated Hospital of Zhejiang University of Traditional Chinese Medicine from Apr 2019 to Apr 2020 were recruited as the study subjects,the changes of endoscopic features were observed after the anti-Hp treatment,and the expression levels of miR-1296-5 p and miR-155-5 p in gastric mucosal tissues were detected by means of reverse transcription-polymerase chain reaction(RT-PCR).RESULTS The percentages of the CAG patients with variegated changes,vascular exposure,mucosal granules,intestinal nodules,mucosal erosion and intramucosal hemorrhage were significantly lower after the treatment than before the treatment(P<0.05).The expression levels of miR-1296-5 p and miR-155-5 p in gastric mucosal tissues were significantly higher after the treatment than before the treatment(P<0.05).The percentages of the patients with gastric mucosal atrophy,chronic inflammation,and moderate to severe intestinal metaplasia were relatively high among the patients with low expression of miR-1296-5 p(P<0.05);the percentages of the patients with moderate to severe chronic inflammation were relatively high among the patients with low expression of miR-155-5 p(P<0.05).The area under curve(AUC)of joint detection of miR-1296-5 p and miR-155-5 p was 0.857 in prediction of poor prognosis of the CAG patients complicated with Hp infection,significantly higher than that of the single detection of miR-1296-5 p or miR-155-5 p.CONCLUSION The endoscopic manifestations of the CAG patients complicated with Hp infection can be improved after the anti-Hp treatment,both miR-1296-5 p and miR-155-5 p show low expression state in the atrophic gastric mucosal tissues,and the joint detection can raise the efficiency in prediction of poor prognosis.
