管电流调制技术联合低管电压低对比剂剂量与浓度在CT冠状动脉成像中的可行性研究 被引量:5

摘要 目的探究管电流调制技术联合低管电压、低对比剂浓度与剂量在CT冠状动脉血管成像(CCTA)中的可行性。方法将92例(男性44例、女性48例)因冠状动脉粥样硬化性心脏病(冠心病)筛查行CT冠状动脉成像的患者随机分为2组,A组45例采用管电流调制技术自适应mAs,100 kV,45 mL(320 mgI/mL)对比剂用量进行血管成像,B组47例采用固定管电流技术230 mAs,120 kV,60 mL(350 mgI/mL)对比剂用量进行血管成像。记录2组患者回旋支、前降支、右冠状动脉层面CT值、冠状动脉图像噪声及检查成功率并计算图像对比信噪比(CNR)、信噪比(SNR)、有效剂量(ED)。结果2组图像皆满足诊断要求,2名评分者对2组CCTA图像评分一致性优良(Kappa=0.916、0.924),2组所有患者剂量长度乘积(DLP)、CT容积剂量指数(CTDIvol)、ED、回旋支、前降支、右冠状动脉层面CT值、CT冠状动脉图像噪声均具有统计学意义(P<0.01)。2组年龄、性别、检查成功率、SNR、CNR、BMI差异无统计学意义(P>0.05)。结论管电流调制技术联合低管电压、低对比剂剂量与浓度在CT冠状动脉成像中可获得满足诊断的图像,虽然未提升图像CNR,但明显降低了患者所受辐射剂量与对比剂使用量,且不对检查成功率造成影响。 Objective Explore the feasibility of tube current modulation technology combined with low tube voltage,low contrast agent concentration and dose in CT coronary artery imaging.Methods Ninty-two patients(44 males and 48 females)who underwent CT coronary artery imaging due to coronary heart disease screening were randomly divided into two groups.Group A(n=45)used tube current modulation technology to adapt mAs,100 kV,45 mL(320 mgI/mL)contrast medium dosage for angiography.Group B(n=47)used fixed tube current technique,230 mAs,120 kV,60 mL(350 mgI/mL)contrast medium for angiography.The CT value,coronary artery image noise and success rate of the two groups were recorded,and the contrast-to-noise ratio(CNR),signal-to-noise ratio(SNR)and effective dose(ED)were calculated.Results Both sets of images met the diagnostic requirements.The two scorers had excellent consi-stencyin the scores of the two groups of whole abdominal CTA images(Kappa=0.916,0.924).The dose length product(DLP),CT volume dose index(CTDIvol),ED,circumflex artery,anterior descending artery,CT value of right coronary artery slice and CT coronary artery image noise of all patients in the two groups were statistically significant(P<0.01).There were no significant differences in age,gender,success rate,SNR,CNR and BMI between the two groups(P>0.05).Conclusion Tube current modulation technology combined with low tube voltage low contrast agent dose and concentration can obtain images that satisfy the diagnosis in CT coronary artery imaging.Although the image CNR is not improved,it significantly reduces the radiation dose and contrast medium used by the patient,and does not affect the success rate of the examination.
