摘要
[目的]探讨旋顶距和颈干角在髋关节置换术中对肢体长度及偏心距的影响。[方法] 2019年6月~2020年12月单侧髋关节置换的患者117例纳入本研究。依据术后标准双髋正位X线片中测量健侧颈干角与假体颈干角差值,将患者分为颈干角正常、减小和增大三组,比较三组间健侧与假体侧各测量指标差异。[结果]三组间偏心距和旋顶距健侧与假体侧差值的差异有统计学意义(P<0.05),而三组间旋泪距健侧与假体侧差值的差异无统计学意义(P>0.05)。[结论]颈干角对肢体长度和偏心距有重要影响。健侧旋顶距联合颈干角可以为髋关节置换术中准确的控制肢体长度和头颈偏距提供有效的个体化指导。
[Objective] To investigate the effect of rotation center-greater trochanter tip distance and neck-shaft angle on limb length and offset in total hip arthroplasty. [Methods] A total of 117 patients who had unilateral hip replacement performed from June 2019 to December 2020 were included in this study. According to the neck-shaft angle difference between the uninvolved side and the prosthetic side measured in the postoperative standard bilateral hip anteroposterior film, the patients were divided into three groups, the normal neck-shaft angle group, the decreased neck-shaft angle group, and the increased neck-shaft angle group. The differences in the measurement items between both sides were compared among the 3 groups. [Results] There were statistically significant differences among the 3 groups in the bilateral differences of the offset and rotation center-greater trochanter tip distance(P<0.05), although no statistically significant differences were found regarding to the bilateral differences in the distance between rotation center and connecting line of bilateral teardrops(P>0.05).[Conclusion] The neck-shaft angle has an important influence on the limb length and offset. The rotation center-greater trochanter tip distance combined with neck-shaft angle might be used as a reference for accurate control of limb length and head-neck offset on individualized basis in total hip arthroplasty.
