摘要
目的探讨浓度≤0.5%碘伏溶液护理尿道口对导尿管相关尿路感染(CAUTI)的预防效果。方法检索CNKI、万方数据库、VIP数据库,搜集使用浓度≤0.5%碘伏溶液护理尿道口预防CAUTI的随机对照试验(RCT)数据,检索时间均为数据库建立日至2020年2月20日。两位评价员独立筛选文献、提取资料,评价偏倚风险后,使用RevMan5.3软件进行Meta分析。结果本研究纳入8个RCT,共1289例置管患者。Meta分析显示:在导尿0~3 d、5~7 d、7~14 d观察组与对照组预防CAUTI发生上差异无统计学意义。亚组分析发现,神经科及重症监护室(ICU)患者使用碘伏护理尿道口预防CAUTI效果优于对照组;而泌尿外科、骨科和其他科室预防效果无统计学差异。另外,使用碘伏护理尿道口产生的不良反应更频繁。结论在尿道口护理中使用浓度≤0.5%的碘伏溶液对CAUTI的预防总体上没有明显效果;使用碘伏护理尿道口出现不良反应的可能性较高。
Objective To explore the preventive effect of urethral nursing by iodophor solution with concentration≤0.5%to catheter-related urinary tract infection(CAUTI).Methods Searching databases CNKI,Wanfang and CQVIP to identify the clinical randomized controlled trials of nursing urethral orifices with iodophor solution(concentration≤0.5%)in China from inception to February 20,2020.Two reviewers screened literature,extracted data,and assessed the risk of bias of included trials independently.Then RevMan 5.3 software was used to conduct Meta-analysis.Results A total of eight RCTs involving 1289 patients with indwelling catheter were included.The Meta-analysis results showed no statistically significant differences between the iodophor group and the control group in preventing CAUTI in 0-3 d,5-7 d,and 7-14 d.Subgroup analysis showed that the effect of using iodophor to nurse urethra orifice was better than that of the control group for patients in neurology department and Intensive Care Unit(ICU)department to prevent CAUTI.However,in urology department,orthopedics department or others,there were no significant differences.In addition,the iodophor solution group has more adverse reactions compared with control group.Conclusion Iodophor solution with concentration≤0.5%showed no significant positive effects on the prevention of CAUTI in general.Using iodophor to nurse urethral orifice is more likely to have adverse reactions.
