摘要
目的探讨能谱CT碘基图在肺癌患者放化疗疗效评估中的应用价值。方法依据实体瘤疗效评价标准1.1版将84例肺癌患者分为有效组(n=59)和无效组(n=25),所有患者放化疗前后均接受能谱CT成像扫描,观察并分析能谱CT相关参数,包括肿瘤最大径、CT值、碘密度值(IC)和能谱曲线斜率(λHU)。采用受试者工作特征(ROC)曲线和曲线下面积(AUC)评估能谱CT参数对肺癌患者放化疗疗效的诊断价值。结果放化疗前,两组患者肿瘤最大径、CT值、动静脉期IC、动静脉期λHU比较,差异均无统计学意义(P﹥0.05)。放化疗后,有效组患者肿瘤最大径、CT值、动静脉期IC、动静脉期λHU均明显低于本组放化疗前和无效组患者,差异均有统计学意义(P﹤0.01)。动静脉期IC及动静脉期λHU联合诊断肺癌患者放化疗疗效的AUC为0.96,高于能谱CT各参数单独检测的AUC,差异均有统计学意义(P﹤0.05)。结论能谱CT通过定量分析肺组织摄碘分布及含量,反映肺癌患者放化疗后肿瘤内微循环的变化,对评价放化疗疗效具有较高的价值。
Objective To explore the application value of energy spectrum CT iodine overlays in the evaluation of the efficacy of radiotherapy and chemotherapy in patients with lung cancer.Method According to the response evaluation criteria in solid tumor 1.1 version,84 patients with lung cancer were divided into effective group(n=59)and ineffective group(n=25).All patients received energy spectrum CT imaging scans before and after radiotherapy and chemotherapy,observe and analyze energy spectrum CT related parameters,including tumor maximum diameter,CT value,iodine concentration(IC)and spectral HU curve(λHU).The receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to evaluate the diagnostic value of energy spectrum CT parameters for the efficacy of radiotherapy and chemotherapy in patients with lung cancer.Result Before the treatment,the differences in maximum diameter of tumors,CT values,arteriovenous phase IC andλHUwere not statistically significant between the two groups(P>0.05).After radiotherapy and chemotherapy,the maximum tumor diameter,CT value,arteriovenous phase IC,and arteriovenous phaseλHUof the effective group were significantly lower than those of the pre-radiation and chemotherapy and the ineffective group,and the differences were statistically significant(P<0.01).The AUC of the combination of IC andλHUin the arteriovenous phase in the diagnosis of lung cancer patients with radiotherapy and chemotherapy was 0.96,which was significantly higher than the AUC detected by each parameter of energy spectrum CT alone,and the differences were statistically significant(P<0.05).Conclusion Energy spectrum CT can quantitatively analyze the distribution and content of iodine uptake in lung tissues,and reflect the changes in tumor microcirculation after radiotherapy and chemotherapy in patients with lung cancer.It is of high value for evaluating the efficacy of radiotherapy and chemotherapy.
