摘要
目的通过与常规12导联心电图进行波形对比,验证单导联在诊断心律失常中的准确性,并利用单导联筛选老年人群中的心房颤动(简称房颤)患者。方法连续入选2019年9月至2019年11月入住上海第九人民医院心内科患者共107例,先行常规12导联心电图检查,随后立即行单导联检查,将两者所获得的心电数据进行对比。并应用该单导联对上海某小区370位老年人进行房颤筛查。结果单导联与常规12导联心电图对房颤、窦性心动过速、窦性心动过缓诊断完全一致,而对房性早搏、室性早搏诊断两者不完全一致,房性早搏3例一致、1例不一致,室性早搏诊断4例一致,2例不一致。应用单导联心电设备对社区老年人群进行房颤筛查,一共检出房颤患者26人,占总人数7.02%,其中首次检出房颤患者14人,占比3.78%,新检出房颤人数占总房颤患者53.85%。结论单导联在心律失常检测中的准确性高,在房颤筛选中具有较高的应用价值。
Objective To verify the accuracy of the single-lead electrocardiogram(ECG) in the diagnosis of arrhythmia by comparing the waveform with the routine 12-lead ECG, and to screen the elderly patients with atrial fibrillation(AF) by using the single-lead ECG. Methods A total of 107 patients admitted to the Department of Cardiology of Shanghai Ninth People′s Hospital from September 2019 to November 2019 were selected consecutively. Routine 12-lead ECG examination was performed first, followed by single-lead ECG examination immediately, and the ECG data obtained from the two were compared.The single-lead ECG was used to screen 370 elderly people for AF in a residential area in Shanghai. Results The diagnosis of AF, sinus tachycardia and sinus bradycardia by single-lead ECG and routine 12 single-lead ECG were completely consistent, while the diagnosis of atrial and ventricular premature was not completely consistent. The diagnosis of atrial premature was consistent in 3 cases and inconsistent in 1 case. The diagnosis of ventricular premature was consistent in 4 cases and inconsistent in 2 cases.A total of 26 patients with AF were detected, accounting for 7.02% of the total population, with 14 patients with AF detected for the first time, accounting for 3.78% of the total population, and 53.85% of patients with AF were newly detected. Conclusion The accuracy of single-lead ECG is high in the detection of arrhythmia and it has high application value in the screening of AF.[Chinese Journal of Cardiac Pacing and Electrophysiology,2021,35(3):253-255]
