摘要
【目的】探讨鼻内镜下应用低温等离子开窗术治疗上颌骨囊肿的疗效。【方法】收集我院2010年1月至2015年12月上颌骨囊肿患者,根据手术方式不同分为两组,每组各25例,对照组患者采取传统手术方案,观察组实施鼻内镜下应用低温等离子开窗术。比较两组患者术中出血量、创面愈合时间、疼痛程度评分、患者生活质量改善、并发症发生率、复发率的差异。【结果】治疗前两组生活质量比较无明显差异(P>0.05),治疗后观察组生活质量评分高于对照组(P<0.05),提示观察组生活质量改善要优于对照组。观察组患者术中出血量(11.07±4.12)ml少于对照组(92.16±10.13)ml,术后创面愈合时间(8.01±2.21)d短于对照组(12.15±3.17)d,患者疼痛评分(2.25±1.21)要低于对照组(6.65±2.59),均P<0.05。观察组术后并发症发生率低于对照组(P<0.05),复发率要低于对照组。【结论】鼻内镜下应用低温等离子射频消融开窗术治疗上颌骨囊肿属于微创手术,具有术后并发症少、复发率低、出血量少、创伤小、术后恢复快、患者痛苦轻等优点,尤其适合侵犯鼻腔底面颊部、硬腭隆起的上颌骨囊肿。选择合适病例的情况下可作为首选手术方法,值得临床推广。
【Objective】To investigate the effect of application of cryo-plasma fenestration under nasal endoscopy on maxillary cysts under nasal endoscopy. METHODS: The patients with maxillary cysts from January 2010 to December 2015 were collected and divided into two groups according to different surgical methods. Each group consisted of 25 patients. The control group underwent conventional surgical procedures. The observation group underwent endoscopic application of low temperature plasma. Window opening. The difference of intraoperative blood loss, wound healing time, pain degree score, patient quality of life improvement, complication rate and recurrence rate were compared between the two groups. RESULTS: There was no significant difference in quality of life between the two groups before treatment. The quality of life score of the observation group was higher than that of the control group, which suggest that the improvement of quality of life in the observation group was better than that of the control group. The intraoperative blood loss was lower in the observation group than in the control group, and the wound healing time was shorter than that in the control group.(2.25±1.21 points) was lower than the control group, both P<0.05. The incidence of postoperative complications in the observation group was lower than that in the control group, and the recurrence rate was lower than that of the control group. Conclusion: Endoscopic application of low-temperature plasma radiofrequency ablation fenestration for the treatment of maxillary cysts with accurate, minimally invasive,less postoperative complications, low recurrence rate, short operation time, minimal bleeding, small trauma, rapid postoperative recovery, patients The advantages of pain and lightness are easy to observe the surgical cavity after surgery, especially for the maxillary cyst which invades the buccal and hard palate of the nasal cavity. It can be used as the preferred surgical method when selecting the appropriate case, which is worthy of clinical promotion.
