摘要
目的评估基于双源CT双能量扫描模式低管电压联合迭代重建(SAFIRE)技术诊断泌尿系结石的可行性。方法采用双源CT双能量扫描模式对56例疑诊泌尿系结石患者行全腹部CT平扫。设定权重因子M为0.5、1.0,采用线性融合技术获得类似于标准管电压(120 kVp)的融合图像(A组)和低管电压(80 kVp)图像(B组),分别行滤波反投影重建(FBP)及SAFIRE。以A组图像作为参考标准,比较2组图像所示结石数目、大小、CT值和肾实质CT值及噪声,并比较其图像质量评分及肾实质信号噪声比(SNR)。结果A、B组均检出189枚结石,其中肾结石136枚,输尿管结石51枚,膀胱结石2枚。A组与B组间结石大小[0.86(0.57,1.39)cm vs.0.85(0.59,1.34)cm]、肾实质噪声[(11.64±2.30)HU vs.(11.81±2.29)HU]及肾实质SNR[(3.09±0.72)vs.(3.08±0.73)]、图像质量评分[5.00(5.00,5.00)分vs.5.00(4.63,5.00)分]差异均无统计学意义(P均>0.05),结石CT值[(802.67±398.08)HU vs.(1003.18±513.98)HU]及肾实质CT值[(34.55±3.18)HU vs.(34.92±3.91)HU]差异均有统计学意义(P均<0.05)。容积剂量指数和辐射剂量长度乘积分别为(12.28±2.38)mGy及(530.86±119.25)mGy·cm。结论双源CT双能量扫描模式低管电压联合SAFIRE图像质量与标准管电压图像大致相当,可用于诊断泌尿系结石。
Objective To observe the feasibility of low tube voltage combined with sinogram affirmed iterative reconstruction(SAFIRE)based on dual source CT dual energy scanning mode for urinary calculi.Methods Totally 56 patients with suspected urinary calculi underwent whole abdomen plain CT examination with dual source CT dual energy scanning mode.Setting the weighting factors as M=0.5 and 1.0,fusion images(group A)similar to the standard tube voltage(120 kVp)and low tube voltage images(80 kVp,group B)were obtained with linear blending technique,and then filter back projection reconstruction(FBP)and SAFIRE were performed,respectively.Images of group A were used as reference standard,the number,size and CT value of urinary calculi,CT value and noise of renal parenchyma,signal to noise ratio(SNR)of renal parenchyma and image quality score were compared between groups.Results Totally 189 urinary calculi were found in both group A and B,including 136 kidney stones,51 ureteral stones and 2 bladder stones.The calculus size(0.86[0.57,1.39]cm vs.0.85[0.59,1.34]cm),noise of renal parenchymal([11.64±2.30]HU vs.[11.81±2.29]HU)and SNR of renal parenchymal([3.09±0.72]vs.[3.08±0.73]),the image quality score(5.00[5.00,5.00]vs.5.00[4.63,5.00])of group A and group B were not significantly different(all P>0.05).CT value of calculi([802.67±398.08]HU vs.[1003.18±513.98]HU)and CT value of renal parenchymal([34.55±3.18]HU vs.[34.92±3.91]HU)were both significantly different in group A and group B(both P<0.05).The CT dose index volume and dose-length product of patients were(12.28±2.38)mGy and(530.86±119.25)mGy·cm,respectively.Conclusion Image quality of low tube voltage combined with SAFIRE based on dual source CT dual energy scanning mode was almost the same as that of standard tube voltage,which could be used for urinary calculi.
